2014425 Primary Facility 2011-10-25 Routine/Complete Inspection Pending DEP arrived on site at 11 am. The purpose of the inspection was to check the status of the wells and not any activity on site. At the time of inspection, crews were installing new plastic secondary containment for upcoming frac. Six wells have been drilled (1H - 6H). All six wells are currently shut in and not vented. Inspection of the Baker 4H cellar revealed steady bubbling between the 9 5/8" and 13 3/8" casing strings. It is requested by the DEP that the Operator identify the source of the leak. The Operator is to notify the DEP when this work is to be completed. The Operator shall provide a summary of the findings, and a description fo what repairs need to be completed. According to Chapter 78 Section 78.86...the Operator has the option to correct the defect or submit a plan to correct the defect for approval by the Department within 30 days.

2041359 Primary Facility 2012-02-07 Follow-up Inspection Pending DEP on site 10 am to follow up on post fraccing incindent. Met with Steve Osborne and Chris Kyea of Carrizo. As per meeting with DEP and Carrizo, operator was to change assembly that failed on well heads and have Boots N Coots perform audit on well heads and frac equipment. New Companion flanges have been installed (molded) with true 10000 psi rating. Operator has re-torqued all bolts, cleaned grease pans on pumps, and cleaned up any spills or fluid associated with intial fraccing incident. Boots n Coots will be on site on Feb 8th to do audit. DEP will return on Feb 8th. At this time, no activity shall be performed at the site until approval from Department.

Another B&C inspector will be on site later today to perform more Q5 inspection on well heads. 2414155 Primary Facility 2015-10-08 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 11:30 a.m., the wells were shut in with a crew working on the production line, met with Carrizo representatives Joe Sodon and Chris Lewis, Joe and Chris pumped water into the cellar to determine where the gas bubbles detected from the inspection on 4/1/15 were coming from. The water was pumped to a level above the (13x9) annulus with no gas bubbles detected. It was determined that the gas bubbles were coming from the (9x5) annulus, the (9x5) vent pipe was open and venting to a tank, there was 709psi on the production casing and 686psi on the production line. I took a walk around the site and everything looked OK.

2511743 Primary Facility 2016-08-17 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination on the Baker well pad (ESX10-115-0029 (1)). The wells associated with the pad are as follows:

The site was rocky, but where there was enough soil for growth, vegetation was meeting or exceeding the 70% vegetative standard.  There were a few remaining BMPs (silt sock) and these were promptly removed by Carrizo employees.  Off the access road to the Impoundment and Well Site the landowner constructed an earthwork gun range, well vegetated and stable.  This is causing no impact to the site or access road.  
The area was stable and had no erosional issues at this time and it is recommended that this Notice of Termination be APPROVED.

2512484 Primary Facility 2016-08-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m., the cellar had water at the bottom with no bubbling noticed; the (9x5) vent pipe was open and venting into a tank. There was 493psi on the production casing and 1132psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:40p.m.

2606525 Primary Facility 2017-06-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:35a.m.

The cellar was full of water with bubbling noticed. Gas was detected at 100% LEL and peaked at 16% methane. It was determined that the bubbling was coming from the (9x5) annulus during an inspection (IR#2414155) conducted on 10/8/15. The (9x5) annulus vent pipe was open and venting into the production tank. There was 459psi on the production casing and 450psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:20a.m.

2713027 Primary Facility 2018-03-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2817034 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there was no gas detected with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. The permit number for the 4H on the entrance sign was incorrect (see photo #2). It was 155-20468 and should be 115-20468. I contacted Jesse Bonnice of BKV to have it corrected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2851757 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted I performed a Routine Complete (RTNC) inspection of the Baker Well Pad on March 11, 2019. The weather was sunny and relatively warm, around 38 degrees.

The access road was a bit muddy but not unstable. The access road is quite long, passing some small logging operations, as well as a target shooting range situated along its length.

The well pad surface was plowed and mostly wet but stable. The wellheads were encased in a carport structure. They were surrounded by clean gravel and were properly labeled.

The containment surrounding the production tanks and apparatus was mostly empty and had sufficient freeboard.

The cut and fill slopes of the site were rocky and stable. Where there was sufficient soil, there was adequate vegetation. No areas of instability were noted.

The impoundment on site was full to capacity. It would be recommended to draw down the impoundment to prevent overflow and possible resulting E&S issues.
