1993398 Primary Facility 2011-07-21 Drilling/Alteration No Violations Noted Spill occured 1930 on Tues, 7/19/2011. Approximately 40 gal of OBM (ABS 40). They were tripping pipe, lifted the pipe slowly and mud in the pipe ran out and splattered on BOP stack and on mats below.They sucked up mud with a vacuum, and put in tank with cuttings, Wayne Simon called immediately and spoke to Mr. Bisignani with emergency response. All of spill was on containment. I checked area and ther is still some mud, which they are still cleaning(on BOP). Skidding rig Wed. morning- working on the 3 H.

2517573 Primary Facility 2016-09-08 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40p.m., the cellar was empty; the (9x5) vent pipe was open and venting into a tank. There was 497psi on the production casing and 442psi on the production line. I took a walk around the location and noticed what appeared to grease in a water puddle. I contacted Jesse Bonnice of Carrizo to suggest it be cleaned up.

I left the site at 13:15p.m.

2528710 Primary Facility 2016-10-03 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination (NOT) for the Baker North well pad (ESX10-115-0080). The wells associated with the pad are as follows:

115-20536 115-20538 115-20539 115-20540 The site was stable and the vegetation was primarily perennial grasses. The vegetation met or exceeded the 70 percent vegetative standard. There were no stability problems to report, and the site was well constructed. There were a few third party trails intercepting the well site, however there were no erosional issues associated with these trails.

I recommend APPROVAL of this Notice of Termination for the Baker North Well Pad.

2606528 Primary Facility 2017-06-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:55a.m.

The cellar was empty. The (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 470psi on the production casing and 449psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:35a.m.

2622516 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the Baker North Well site on July 20, 2017. The site was stable at the time of inspection. The access road leading in had no washing or erosional issues and the pad surface was solid with no excessive ponding or mud issues. The vegetation was well established and healthy looking.

There were no violations noted.

2714391 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2817676 Primary Facility 2018-12-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The production tanks for this site are located on the Baker pad. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
