I took a walk around the location and everything looked OK.

2509587 Primary Facility 2016-07-28 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Baker North well pad (ESX10-115-0080). The wells associated with the pad are as follows:

The site was stable and well vegetated with no apparent erosional issues.

2517574 Primary Facility 2016-09-08 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40p.m., the cellar had water at the bottom with no bubbling was noticed; the (9x5) vent pipe was open and venting into a tank. There was 759psi on the production casing and 427psi on the production line. I took a walk around the location and noticed what appeared to grease in a water puddle. I contacted Jesse Bonnice of Carrizo to suggest it be cleaned up.

I left the site at 13:15p.m.

2606529 Primary Facility 2017-06-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:55a.m.

The cellar was about a quarter full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 521psi on the production casing and 460psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:35a.m.

2622518 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the Baker North Well site on July 20, 2017. The site was stable at the time of inspection. The access road leading in had no washing or erosional issues and the pad surface was solid with no excessive ponding or mud issues. The vegetation was well established and healthy looking.

There were no violations noted.

2715661 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2817680 Primary Facility 2018-12-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The production tanks for this site are located on the Baker pad. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
