2016136 Primary Facility 2011-11-01 Drilling/Alteration No Violations Noted DEP on site 12:30. Patterson Rig 261. Drilling out surface casing. Will be doing FIT on formation for 30 mins 15 ppg EMW. Walked site with Co. Man. Secondary containment in place. Have mats on solids handling area. REst of area lined with 60 mil plastic. Checked cellar - no signs of gas shows at surface. No violations observed. DEP offsite 13:00.
2031996 Primary Facility 2012-01-05 Drilling/Alteration No Violations Noted On site 10 am. Mercer Service Rig on site...set up on well. Rigging up to perforate Stage 1. Will also be perforating Stage 1 of S Sus 2H. Met Company man on site. No noticeable leaks at well head or bubbling in cellar. Secondary containment in place.

2441940 Primary Facility 2016-01-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:25 p.m., the cellar was full of ice, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 0psi on it and the valve going to the tank was closed. There was 125psi on the production casing and 908psi on the production line.

I left the site at 12:55 p.m.

2480413 Primary Facility 2016-05-03 Complaint Inspection No Violations Noted
2481788 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:05 a.m., met with Marc Millar (Lease Operations Specialist for Chesapeake). The cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and the valve going to the tank was closed, there was 141psi on the production casing and 997psi on the production line. Marc took the plug out of the side of the multiport valve and opened the valve so I could check for gas with an Altar 5X gas meter that was calibrated on 4/22/16. The valve going to the vent tank on the (13x9) annulus was closed during this process.

I left the site at 10:45 a.m

2734174 Primary Facility 2018-05-24 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the GB well site. The wells associated with this site are as follows: 115-20618(plugged), 115-20619(plugged), 115-20648(plugged), 115-20650 (plugged), 115-20647, 115-20649. The active wells were properly labeled.

The access road and pad surface were stable and solid with active farming occurring alongside. There were a few wet areas on the pad due to recent rain.

The slopes surrounding the elevated pad were well vegetated except in areas too rocky to permit good growth. There did not appear to be any areas of erosional issues occurring.

There were no violations observed at the time of inspection.
