2073372 Primary Facility 2012-04-27 Routine/Complete Inspection No Violations Noted During the investigation, I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen ponded water inside the duck pond containment area where the on-site mobile pretreatment system to filter frac flowback fluid is located. Conductivity, Salinity and TDS readings indicated over the limit, with the limit on the meter being 19.99 milliSiemens/cm. A vacuum truck, which was on-site during the inspection, quickly drove over and removed the water from inside this containment area. The duck pond containment, where the filtering equipment and pump are housed, is located on the containment liner that was installed for drilling and hydrofracturing operations.

2375456 Primary Facility 2015-05-26 Incident- Response to Accident or Event No Violations Noted The Department arrived on-site at 13:00 in response to a release of an unknown volume of production fluid and the discovery of dead vegetation associated with the release. Carrizo has recently been visiting the site on a regular basis due to an erosion problem observed during an inspection conducted by the Department on 4/7/2015. The Department issued a Notice of Violation to Carrizo on 4/22/2015 due to failure of the cut slope. As the Department arrived, Carrizo was in the process of commencing repairs to the cut slope. Gary Byron, consultant of Carrizo, notified the Department via e-mail on 5/22/15 of Carrizo’s observance of an area of dead vegetation off the Southeast end of the pad. The dead vegetation pathway was back tracked to an area where (4) production fluid tanks were stored on secondary containment. Compromised liner integrity was discovered in the containment staging the production fluid tanks. Carrizo believes a small release of had occurred following storm events on 5/17/15 and 5/18/15. The release appears to exceed (42) gallons.
2422349 Primary Facility 2015-10-28 Routine/Complete Inspection No Violations Noted The Department, represented by Frank Hoehle, was on site at the Shaskas well pad to conduct a routine complete inspection. The wells associated with this site are as follows:
2464785 Primary Facility 2016-03-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35 a.m., the cellar had water at the bottom with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was about 710psi on the production casing and about 630psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 12:15 p.m.

2548732 Primary Facility 2016-12-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:35a.m., the cellar had ice at the bottom, the (9x5) vent pipe was open and venting into the production tank. There was 603psi on the production casing and 463psi on the production line. I took a walk around the location, there were twenty 500bbl tanks that were labeled fresh and on containment, the containment appeared to be in good condition. There were no obvious concerns noticed.

I left the site at 11:15a.m.

2636985 Primary Facility 2017-09-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed.

2661549 Primary Facility 2017-11-13 Routine/Complete Inspection No Violations Noted I inspected the Shaskas South well pad for a Routine Complete inspection on November 13, 2017. The access road was not muddy and the pad surface was solid with some puddling due to recent rains. The cut and fill slopes were stable, solid and well vegetated at the time of inspection and there were no apparent erosional issues. There were no violations noted.
2677577 Primary Facility 2017-12-07 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:53. The weather conditions were partly cloudy skies and breezy with temps in the mid 30's. I initially met Roy Birdwell, Superintendent of Hart Maintenance and Construction. Mr. Birdwell informed me of the following activities in progress. "We are in the process of welding pipe that will be run from the production fluid tanks containment to near the wells and evaporator unit. A compressor will be installed to increase pressure from (300) to (600) psi for all wells here. Frac will start soon on a nearby pad. The pressure is (600) psi at the wellheads on that pad. I am welding pieces of pipeline, which will be tied to the GPU's. A (6) inch suction line will go to the compressor. I have been working here over a week now and the evaporator has not run. I did not see any trucks haul water out of the frac tanks. The unit may be hauled out soon."

I met with Jordan Brand of Carrizo, Meter Technician, midway through the inspection. Mr. Brand stated the following. "The Evaporator has not b

2744165 Primary Facility 2018-06-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2767915 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted I inspected the Shaskas well site following a report by BKV detailing a slip on the cut slope, noticed during an after-rain inspection. The cut slope had previously experienced a slide however that repaired area was solid and stable. The new slip area occurred during the torrential rainfall which swept the region. The slip was not large and was scheduled to be repaired as soon as possible. No waters were impacted and the edge of the pad was not touched.

The rest of the cut and fill slopes were well vegetated and there was no evidence of erosional issues noted.

The access road was solid and stable as was the pad surface.

The wells were properly labeled.

There were no violations noted.

2839507 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
