Drillers log on site and up to date. 20 " set at 46'. 13 3/8 will be set at 638'. Will be running FIT after running surface casing. Secondary containment in place. Some tears by rolloffs. operator will repair. Also need to berm up back of location. Operator indicated the berms are in process of being built and will be finished once all equipment is moved onto containment. No violations observed.
2022395 Primary Facility 2011-11-22 Follow-up Inspection Pending Follow up inspection. Operator reported bubbling in cellar. Well is currently shut in. Floor of cellar is poured concrete. The top of conductor and surface string are below the concrete floor. A small steady stream of bubbling is present coming through the concrete floor approximately 3" away from 9 5/8" casing. Gauge on annulus reads 0 psi (3000 lb). No activity on site at time of inspection. Operator indicated they will be removing concrete and exposing tops of casing strings to identify source of bubbling.
2027611 Primary Facility 2011-11-30 Drilling/Alteration Pending DEP on site 14:00. Met with Mr. Bell - drilling supt. Bubbling reported in cellar of well while on site marking location of next well. On 11/2/11 an inspection of the well confirmed the bubbling in the cellar. The cellar floor at that time was cemented. The bubbling observed consisted of a small stream of steady bublng coming up through cement approximately 3" away from the 9 5/8" casing string. The 13 3/8" and 20" casing tops were under concrete at that time. Operator has removed cement and exposed tops of casings to identify source of bubbling. Operator indicated 3% LEL. CO2 ranged from 60 - 80 ppm. At time of this inspection a pump truck was on site and removed fluid from cellar. Observed minor bubbling on 9 5/8" annulus. Operator indicated a bond log will be run next week and all data collected will be shared with the DEP. No violation exists at this time. Operator has 30 days to correct the defect or present a plan to fix the defective cement.
2031216 Primary Facility 2012-01-03 Follow-up Inspection Pending Gas bubbling reported and confirmed on 9 x 13" annulus during previous inspection. Operator installed a packoff (cap) on well on 12/13/11. When installed, Operator reports no bubbling at that time. The annulus was shut in and pressure monitored. Pressure increased to a moximum of 22 psi after 48 hours. Operator issued repsonse letter with logs to DEP (rec'd 12/30/11). To date, there has been no reported gas leaking on 20 x 13" annulus. At this time, Operator is requesting to vent the leaking annulus and monitor the well without further immediate action. If there is any change to the status of the well, Operator will notify DEP immediately. Operator is planning to mobilixe rig onto the well to continue drilling oeprations in the near future. At this time, there is "pending" status on this well. There is evidence indicating a defective cement job on this string of casing. Operator is aware that future remediation efforts may be necessary.

There was a steel cage around the well. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 606psi on the production casing and 588psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:40p.m.
