2020129 | Primary Facility | 2011-11-17 | Drilling/Alteration | No Violations Noted | DEP on site 12 :17. No activity on site. Site ID sign in place with proper information. Permit posted on site ID sign. Three cellars have been installed. Only 2H currently has conductor drilled. Concrete pad surrounds three cellars on site. |
2031978 | Primary Facility | 2012-01-05 | Routine/Complete Inspection | No Violations Noted | DEP on site 9 am. No rig on site. 3 slots installed. Only 2H well drilled to date. Excavator on site cleaning up plastic around well. Cellar is pumped down. Did not oberve any bubbling at time of inspection. Request Operator to tag wells when move off location. |
2089449 | Primary Facility | 2012-08-06 | Complaint Inspection | No Violations Noted | respond to industrial accident, two workers sand blasted when frac hose ruptured(hospital) |
2253716 | Primary Facility | 2014-03-14 | Routine/Complete Inspection | No Violations Noted | Prod well. Cellar is ice filled. |
2722343 | Primary Facility | 2018-04-24 | Routine/Complete Inspection | No Violations Noted | I performed a RTNC inspection of the Susan well location. The well pad was solid as was the access road. The cut slope was stable and well vegetated. There was evidence of repair on the cut slope but the old repair was stable. The fill slope was solid and well vegetated. The wells were all properly labeled. The wells associated with this site are as follows:
115-20705 115-22446 115-22447. There were no erosional issues noted. There were no violations. |
2754337 | Primary Facility | 2018-06-11 | Routine/Complete Inspection | No Violations Noted | Arrived on location at 10:00. Onsite in response to new spud notices received for the Susan 4H & 5H wells. Upon inspection, (2) new 20" conductors have been set and cemented to surface. (1) existing well, the Susan 2H had previously been producing but has been disconnected from production and temporarily shut-in with a protective cage placed over this well in preparation for drilling of new wells on pad.
- The 2H wells production tree has been removed and replaced with a TA night cap. (3) annular vents were present. The (5/P) vent was shut in to pressure gauge. The (9x5) & (13x9) vents were both open and venting through Sentry vent fittings. Gas checks performed at both vent fittings did not detect any gas present at time of inspection. No obvious concerns were noted at this well at time of inspection. - Liner, Drilling mats and some containment has been set on pad. (7) Baker tanks have been staged on location in secondary containment in anticipation of future drilling of new wells on pad. - Overall, wells |
