2422695 Primary Facility 2015-11-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45, the cellar was empty, the (9x5) vent pipe was open and venting to a tank with a 600psi pressure gauge which was reading 0psi, there was 548psi on the production casing and 712psi on the production line. I took a walk around the location and everything looked OK.
2484391 Primary Facility 2016-05-03 Routine/Partial Inspection No Violations Noted The Department was on site at the Giangrieco Well Site, located at 936 Dark Hallow Road Montrose, PA, to conduct a Routine Partial Inspection. At the time of the inspection, the Erosion & Sediment Visual Inspection Checklist indicated that an inspection was performed on 4/27/16. The slopes consist of entirely rock on the majority of the well site. The areas of the site that are not rock, appear to meet temporary stabilization requirements.

The Department previously inspected the Southwestern Energy Ivey Well Site which the Giangrieco shares a portion of access road with. The Ivey E&S Plan Map indicated that Carrizo’s access road was constructed within a forested wetland. This was noted to be identified on 10/18/11 and it mentioned that Carrizo was notified at the time.

2516423 Primary Facility 2016-08-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:25a.m., the cellar was empty; the (9x5) vent pipe was open and venting into a tank. There was 478psi on the production casing and 0psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 11:10a.m.

2528647 Primary Facility 2016-10-11 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination inspection (NOT) for the Giangrieco A Well Site, ESX11-115-0033. The wells associated with this site are as follows:

The area was stable and had no erosional issues at this time and it is recommended that this Notice of Termination be APPROVED.

2528834 Primary Facility 2016-09-29 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete inspection for the Giangrieco A Well Site, ESX11-115-0033. Department representatives Frank Hoehle and Jon Sassi were in attendance.
The wells associated with this site are as follows:

There was evidence of recent work on the well site where the cellar of 115-20714 was added onto and additions were made to the valving.

Jon Sassi tested areas around and in the containment, with no unusual readings of note. There was healthy vegetation noted near the discharge area, and the soil conductivity in said area was low.

There was no erosion or sedimentation problems occurring at this time. Some areas of the site had low vegetation, however these areas all were extremely rocky, with no fines running downgradient.

There were no violations.

2544065 Primary Facility 2016-10-03 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination inspection for the Giangrieco A Well Site, ESX11-115-0033.
The wells associated with this site are as follows:

The site was stable, and while very rocky, the vegetation that was established was perennial and was up to standard. In some areas, the vegetation was not over the 70% perennial threshold, however this was not due to the lack of ability of the vegetative cover to thrive, it was due to the extremely rocky nature of the soil.

There were no erosion or sedimentation problems occurring at this time.

I recommend APPROVAL of this Notice of Termination.

2608722 Primary Facility 2017-06-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:55a.m.

The cellar was empty. The (9x5) annulus vent pipe was open and venting into the production tank. There was 422psi on the production casing and 382psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:45p.m.

2715679 Primary Facility 2018-04-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there was cement between the (20x13) and (13x9) annuli. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2816452 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was empty and there no was gas detected with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
