2352807 Primary Facility 2015-02-02 Routine/Complete Inspection No Violations Noted Performed inspection for inactive status. Well is a natural gas well with a TD of 11845. The casing lengths and depths are documented on hard copy inspection form. The well is currently shut-in with gauges on all annuli reading 0 psi. The cellar is frozen with ice and several inches of snow. No visible bubbling witnessed. Carrizo is awaiting Regional Infrastructure and plan to put the well into production on or around 2/2020. Inactive status submitted for approval. Approval granted 2/25/2015.

I took a walk around the location and everything looked OK.
2528479 Primary Facility 2016-10-13 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination Inspection of the Frystak Well Pad, ESX11-115-0096. The wells associated with the pad are the:

The Frystak site had all temporary BMPs removed and complied with PCSM and Restoration Plan Option 1. The site was stable and well vegetated with no apparent erosional issues.

I recommend APPROVAL of this Notice of Termination for the Frystak Pad.

2535787 Primary Facility 2016-11-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15a.m., the cellar was about three quarters full of water with no bubbling noticed, the (9x5) vent pipe was open and venting into a tank. I took a walk around the location and there were no obvious concerns noticed.

The well was granted inactive status on 2/25/15. I left the site at 12:20p.m.

2568070 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Department arrived on site to conduct a Routine Complete Inspection. At the time of the inspection, I reviewed the Inspection Checklist. The checklist showed that the most recent inspection was conducted on December 9, 2016 by Bruce Bonnice. He indicated that the filter sock had been removed.
2624161 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:15a.m.

The cellar was full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into the production tank. The valve on the production line and at the GPU were closed. The well is drilled to TD but hasn’t been stimulated. It was granted inactive status on 2/25/15. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:10a.m.

2732546 Primary Facility 2018-05-17 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was granted regulatory inactive status on 2/25/15. There was water 3.5' below grade in the cellar and there was no bubbling noticed, there was no gas detected in the remainder of the cellar. The well hasn't been stimulated but the Department received a pressure test notification on 3/29/18. There were three 500bbl tanks on containment and the containment appeared to be in good condition. The main valve on the production line was closed. I took a walk around the location and there were no obvious concerns noticed.

2772333 Primary Facility 2018-09-07 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company men Larry Gerard and Tom Mattern. Halliburton was on site to frac the 1H, 2H, 3H, 6H and 8H wells. The were running a plug and perf in the 3H and pressure testing iron to start stage 24 on the 2H when I arrived on location. The 1H was completed with 20 stages. There were 23 out of 25 stages completed on the 2H. There were 22 out of 24 stages completed on the 3H. The 6H was completed with 20 stages. The 8H was completed with 19 stages. The water was be pumped to the site from the Bush fresh water impoundment and production water was being trucked to the site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2774392 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted I performed a routine E&S inspection at the Frystack well pad in conjunction with Jon Sassi. The pad had a small slip due to very heavy localized rains, but the fill slope was reworked and stabilized well when I arrived. The area was seeded and mulched as well. The cut slope, pad surface and access road were all in good shape, despite heavy traffic during onsite operations.

There were no violations.

2784064 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted Frank Hoehle, DEP O&G WQS and I arrived at the site at 12:13 to conduct an inspection. The weather conditions are mostly cloudy skies, breezy, warm and humid with temperatures in the mid to upper 70's. Representing BKV at the site is Tom Mattern, Vice President of Operations and Dale Bennett, Site Safety Man. BKV consultants included Kevin Brady of Whitetail Environmental Services and Larry Gerard, Site Company Man. Halliburton is the BKV subcontractor in the process of conducting hydraulic fracturing (stimulation) operations. BKV personnel provided me with following information regarding operations. “Wireline is on the 2H well. Frac is on the 3H well. Perforation casing is on the 2H well. Zipper frac is in progress. Frac operations started on 8/17/18. Frac operations on the 6H and 8H wells have been completed. The 5H and 7H wells were in production, but are currently are shut in. Frac on the 1H well is also in progress. The 4H well has been drilled to total depth but not yet stimulated. We are
2786931 Primary Facility 2018-09-11 Follow-up Inspection No Violations Noted I arrived at the site at 13:11. The weather conditions are mostly cloudy skies, calm winds, and humid with temps in the upper 60's to low 70's. I met with Jesse Bonnice, Senior Production Foreman of BKV, and Kevin Brady, Hydrologist, Environmental Scientist and Wetland Delineator of Whitetail Environmental Services, LLC. The main intent of the follow-up inspection is to evaluate the pad surface and secondary containment on the back side of the pad adjacent to site perimeter berm at the top of the fill slope, due to elevated specific conductance (conductivity) readings measured during the Department's 9/4/18 inspection. Refer to the Department's 9/4/18 inspection report, Inspection ID # 2782158 for additional details.

Frac operations have been recently completed. The next phase of operations to take place at the site is the drilling out of hydraulic fracturing plugs in the he recently stimulated wells, the Frystak 1H-3H, 6H and 8H wells. This will take place in the near future according to Mr. Bonnice

2808719 Primary Facility 2018-11-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The permit numbers on the entrance sign were correct. The well tag was on the well and correct. There was water 1' below grade in the cellar and there was no bubbling noticed. There were twenty eight 500bbl tanks on containment from a recent frac of the 1H, 2H, 3H, 6H and 8H wells. I spoke with Nick Cona of (J P Reilly) and he said 8 of the tanks were being used for production fluid and the rest were empty. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
