Conductor set on 4/3/12 - indicated was dry hole at that time and no shallow gas reported. Conductor cemented with 151 sks of 15.6 ppg cement and set at 75'.
2065322 Primary Facility 2012-05-03 Drilling/Alteration No Violations Noted Nomac 32. Will be circulating tonight and running casing tomorrow morning and cementing early Sat. morning. Walked site with Company Man. Secondary containment in place and in good condition. Once well complete - rig moving to Bradford Cnty. Drillers log on site and up to date. TD of well is 13290'. No violations observed.
2688668 Primary Facility 2018-01-29 Routine/Complete Inspection No Violations Noted I inspected the Jayne SUS well pad on January 29, 2018. The pad surface was slightly soft. The access road was stable and solid. The cut slope and fill slope were adequately vegetated and stable, although past repairs of the cut slope are evident. There did not appear to be any bubbling of the well cellars and the wells associated with this site were properly labeled. The wells associated with this site are the 115-20872, 115-20209 and the 115-20215.

There were no erosional issues apparent. There were no violations noted.
