2078831 Primary Facility 2012-06-21 Follow-up Inspection No Violations Noted Steve Watson and Eric Rooney on site to follow up on bubbling in cellar of 2H and 3H. Operator informed DEP that bubbling had ceased. Operator filled cellar and conductor of 2H while on site. DEP monitored the cellar with Model 60, and Altair gas meters for presence of gas. No detections of gas were observed. No bubbling was observed at time of inspection. DEP asked Operator to notify DEP if bubbles occur while drilling surface section of 2H. While on site I re-inspected areas of pad where violations had prviously been cited. Mr. Gary Byron accompanied me. Secondary containment has been established in the area of the fuel oil tank, and in the drum storage area. All other areas where containment had not been previously established have been installed correctly. At this time no outstanding violations exist.
2105671 Primary Facility 2012-09-24 Follow-up Inspection No Violations Noted On site to grab isotopic gas sample. At time of arrival 9x13" annulus shut in w 100 lb gauge = reading was 42 lbs. Eric Rooney of DEP on site at 9 am to collect sample. Using Altair gas meter detected 100% methane from vent on 9 x 13" annulus. DEP grabbed isotopic gas sample. Operator left vent open upon completion of sampling. Moving Nabors Rig in on site and expect to be drilling later this week.
2110112 Primary Facility 2012-10-10 Drilling/Alteration No Violations Noted Nabors Rig M61. Resumed drilling - e notice recieved on 10/6/12. REsumed drilling on 10/7/12 @ 5570'. i walked site - secondary containment in place consisting of mats and plastic. Drilling ahead on SOBM at time of inspection. BOP charts posted. Site ID sign only had 3 permits posted. 4 wells have been spud. Opeartor must add 4th well permit to sign. Checked cellars - no evidence of bubbling at this time. No violations observed.

2454157 Primary Facility 2016-02-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35 p.m., the cellar had water at the bottom, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting to a tank, there was 1299psi on the production casing and 477psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:20 p.m.

2544391 Primary Facility 2016-10-13 Site Restoration No Violations Noted The Department was on site to inspect the Well Pad, ESX11-115-0138. The wells associated with the pad are as follows:

The site met the 70% vegetative standard, with decent growth everywhere on site. Recently constructed PCSM basins well constructed and well vegetated. No temp. BMPs on site. The vegetation was healthy and the site was stable. There were no erosional issues at this time.

I recommend APPROVAL of this Notice of Termination.

2544896 Primary Facility 2016-12-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45a.m., the cellar was about a quarter full of water with no bubbling noticed, the (13x9) annulus was open and set up with a 100psi pressure gauge with 0psi on it, the (9x5) vent pipe was open and venting into the production tank. There was 744psi on the production casing and 441psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:25a.m.

2545856 Primary Facility 2016-12-15 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 13:15p.m., for a follow up inspection to check the (13x9) annulus for gas show. I was accompanied by Jon Sassi (DEP Environmental Specialist). We met with Joe Fowler from Carrizo, Joe closed the ball valve on the (13x9) annulus vent pipe before removing the pressure gauge. He then opened the valve while I used an Altair 5X gas meter to check for the presence of gas, there was no gas detected when the valve was opened. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:45p.m.

2632988 Primary Facility 2017-09-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed.

2741129 Primary Facility 2018-06-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 0' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836379 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. The (13x9) annulus was shut in with a 30psi pressure gauge with 0psi on it. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
