2091356 Primary Facility 2012-08-01 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 13:00 in response to a release of a corrosion inhibitor (AMD-100) that occurred at 6 PM on 7/31/12. I was accompanied by Ryan Klemish, DEP, O&G, WQS. Mr. Joe Ray Benefield, Company Man, of WPX Energy reported the spill via e-mail to Steve Watson, DEP OGI, at 7:10 PM on 7/31/12 and DEP Northeast Regional Office Emergency Response at 7:15 PM. According to Mr. Benefield’s report, Mr. Benefield pierced a hole in the AMD-100 tote container while attempting to move it from a matting board. The report indicates approximately (20) gallons of the AMD-100 spilled onto the mat board and ran off the board to the surface of the ground on the Well Pad. The spill was cleaned up on 7/31/12. The impacted material was placed into a steel container. Patterson Rig # 332 is the active drilling rig at the site during the inspection, currently drilling the MacGeorge Southeast 1-2H well. The current drilling depth is 1,746 feet.

I took a walk around the location, everything looked OK.

2513758 Primary Facility 2016-08-23 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35p.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 254psi on the production casing and 206psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:10p.m.

2605596 Primary Facility 2017-06-19 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10a.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 5000psi pressure gauge with 0psi on it. There was 202psi on the production casing and about 200psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:55a.m.

2663751 Primary Facility 2017-11-22 Routine/Complete Inspection No Violations Noted I inspected the MacGeorge well pad on November 22, 2017. The access road was firm and stable as was the pad surface. The wells inspected (115-20899, 115-20900, 115-20901) were properly labeled. The cut and fill slopes were solid and stable. All vegetation was healthy and sufficient for the application. There were no apparent erosional issues.

There were no violations noted.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was shut in at the GPU. The cellar was full of gravel and was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
