2233184 Primary Facility 2013-12-11 Routine/Complete Inspection No Violations Noted Mercer Well Service completions rig #34 was on location in the process of drilling out frac plugs and cleaning out the well. Mr. Blanton informed me (17) out of (25) plugs have been drilled out thus far. Operations commenced on 12/10/13. The drill bit has been changed out today and snubbing is in progress. Also, Great Plains is on location breaking down the frac equipment. As frac plugs are in the process of being drilled out, flowback fluid needs to be pumped into the well to carry plug parts out. The flowback fluid is filtered upon returning to the surface and stored in frac tanks and an equalizer tank. The filtration process helps remove the solid and sand material in the flowback. Proflow is the subcontractor performing the flowback filtration of the frac plug drill out fluid. Chemicals are added into a fluid hopper prior to pumping the flowback fluid into the well to reduce friction. A plug catcher is used to catch the solid material from the frac plugs as they are drilled out. All equipment is staged on secondary containment.

2482381 Primary Facility 2016-04-28 Routine/Complete Inspection No Violations Noted On 4/26/16 Eric Hottenstein, Environmental Supervisor of Chesapeake Energy informed Matt Shope, PADEP O&G WQS, via e-mail the following. "We plan on discharging storm water from tank containments this Thursday 4/28/16 starting at 10:00 AM on the Tinna Well Pad in Wyoming County and then heading to the Maris Well Pad in Susquehanna County.” Darrin Chaffee, CHK Water Technician and I arrived at the location at 13:05. Prior to the commencement of discharging the storm water in the production fluid tank containment structure, Mr. Chaffee and I visually assessed the water in the containment for evidence of a visual sheen or discoloration, foam or scum buildup on the surface and a significant amount of sediment. We did not observe sheen, foam or scum buildup on the surface of the water, a significant amount of sediment or any unusual odors in the water. 2714740 Primary Facility 2018-04-05 Site Restoration No Violations Noted I inspected the Maris well site on April 5, 2017. The access road was very solid, and the pad surface was solid as well. The cut and fill slopes were solid and rocky. The wells were labeled correctly. Vegetation was prominent wherever it was possible for it to grow. There were no temporary BMPs left on site. There were no erosional issues noted. There were no observed violations. The wells associated with this site are as follows:

2782145 Primary Facility 2018-10-01 Routine/Complete Inspection No Violations Noted The well access road was firm and solid with no erosional issues. While on site I was approached by the landowner who seemed satisfied with the construction and maintenance of the well pad. The pad surface had some puddling from recent rains but was otherwise solid. The slopes around the well site were stable, but most of the area was simply too rocky to support growth. Where the vegetation was able to grow, it was perennial and exceeded the Department 70% standard. The PCSMs were all in place and the temporary BMPs surrounding the site were all removed. The wells were properly labeled.
