2470972 Primary Facility 2016-04-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45 p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 476psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:20 p.m.

2694283 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Inspection conducted with Herb Swiney, Contract Consultant. Heavy truck traffic on the access road is causing muddy conditions to develop. Please ensure cross culverts and road side ditches are maintained. Crews are on-site actively improving and maintaining stormwater and E&S controls. Fracking operations expected to last about 1 more month. No violations noted.
2700844 Primary Facility 2018-02-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted in response to Cabot's self reporting of a small diesel spill on the tank pad. The spill occurred due to a water truck's saddle tank developing a leak. Cabot reported the spill to be around 18 to 20 gallons. Present during the inspection was Brody Webster, Senior EH&S Specialist. Today's inspection found no visible sign of the released material. Resource Environmental oversaw the remediation of the spill. Container holding the remediated soil still on site. This spill will be considered immediately corrected pending a positive review by the Department of the spill report that Cabot plans on submitting to the Department for review.
2700853 Primary Facility 2018-02-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Present during the inspection was Brody Webster, Cabot EH&S Specialist. As all of the photo's show, the site is experiencing a tremendous amount of mud on the pad surfaces due to the heavy truck traffic occurring as related to the hydraulic fracturing activities occurring at this site. As photos 4 and 8 show, mud is flowing through storm related controls and is being contained by the infiltration berms. As discussed, the Department expects that when conditions improve, the infiltration berms will be reviewed to determine if the high clay content of the mud causes an impairment to the functionality of said berms. No evidence of the mud leaving the site was identified. The Department will continue to monitor this site closely. No violations observed.
