2268568 Primary Facility 2014-04-28 Follow-up Inspection No Violations Noted I met with Zack Morgan of J.P. Reilly and Associates. J.P. Reilly completed the process of repairing the frac tank containment. The sludge treatment operation observed on 4/28/14 and described in the 4/25/14 inspection report, is no longer in operation. The tank containing the sludge has been removed from the location, along with the excavator that transferred the lime into the sludge tank. All other equipment part of the process remains on the location. The secondary containment system as part of the process is still in place. The lime dust impacting the ground surface and stormwater perimeter ditch behind where the sludge treatment operation was staged, has not yet been cleaned up. Mr. Morgan stated that the operation shut down either Friday or Saturday.
2352797 Primary Facility 2015-02-02 Routine/Complete Inspection No Violations Noted Performed inspection for inactive status. Well is a natural gas well with a TD of 10441. The casing lengths and depths are documented on hard copy inspection form. The well is currently shut-in with gauges on all annuli reading 0 psi. The cellar is frozen with ice and several inches of snow. No visible bubbling witnessed. Carrizo is awaiting Regional Infrastructure and plan to put the well into production on or around 2/2020. Inactive status submitted for approval. Approval granted 2/25/2015.

2463349 Primary Facility 2016-03-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:30 a.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) annulus was venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting to a tank, all of the pressure gauges on the well were reading 0psi. I took a walk around the location and everything looked OK.

The well was approved for inactive status on 2/25/15. I left the site at 12:25 p.m.

2528684 Primary Facility 2016-10-13 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination for the Ricci Well Site (ESX12-115-0183). The wells associated with this site are as follows:

The area was stable and had no erosional issues at this time and it is recommended that this Notice of Termination be APPROVED.

2544883 Primary Facility 2016-12-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:45p.m., the cellar had water and ice at the bottom with no bubbling noticed, the (13x9) annulus was open and venting into the atmosphere and no gas was detected, the (9x5) vent pipe was open and venting into the production tank. The well was granted inactive status on 2/27/15. I took a walk around the location and there were no obvious concerns noticed.

2557854 Primary Facility 2017-01-12 Administrative/File Review No Violations Noted The Department conducted an Administrative Inspection in response to receipt of the Well Site Restoration Report received on 10/11/2016. The Department conducted a file review to find that this site had a Restoration Inspection resulting in a Notice of Termination on 10/13/2016.
2630182 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was not in production. It was granted inactive status on 2/27/15. The well has not been stimulated. I took a walk around the location and there were no obvious concerns noticed.

2684043 Primary Facility 2018-01-05 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Floyd Ball. Halliburton was on site to frac the 1H, 2H, 3H, 4H, and 5H wells. They were on stage 17 out of 22 stages on the 3H at time of inspection. There were 11 out of 22 stages completed on the 1H. There were 20 out of 26 stages completed on the 2H. There were 15 out of 18 stages completed on the 4H. There were 10 out of 16 stages completed on the 5H. Fresh water is pumped to the site from the Bonnice fresh water impoundment and production water is being trucked to the site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2693698 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted Jon Sassi and I inspected the Ricci well pad on 2/12/2017. It was a routine complete inspection. Flowback operations were in process at the time of inspection being performed by Foster Energy Services. The site was thawing due to the recent warm temperatures. There was puddling on site due to the thaw. The cut and the fill slopes were stable, as was the access road. There were no erosional issues visible. The wells associated with this site are the 115-21118, 115-21232, 115-21233, 115-21234 and 115-21235. There were no violations observed.
2697989 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted I arrived at 10:54. I was accompanied by Frank Hoehle, DEP O&G WQS during the inspection. We met with Kevin Brady of Whitetail Environmental Services, LLC, Bud Roszel, Facilities Engineer and Dale Bennett both of BKV Operating, LLC. The weather conditions were partly cloudy skies and breezy with temperatures in the 30's. Flowback of the Ricci 1H-5H wells is in progress. Foster Energy Services is the BKV subcontractor conducting flowback operations. The BKV personnel and Mr. Brady explained the following in reference to site activities. "Flowback of the wells started on 2/1/18. The Ricci 1H-5H wells are being flowed back all at once. All of these wells have been stimulated and frac plugs have been drilled out. Each well has its own equipment consisting of a flowback seperation unit, manifold, line heater and sand trap. As the wells are flowed back, natural gas is sent to the gathering pipeline which eventually goes to the sales line. The gathering pipeline flows to the nearby BKV Bonnice Pad. The production e
2698055 Primary Facility 2018-02-22 Administrative/File Review No Violations Noted On 2/16/18 I had submitted an email response to Mr. Roszel and Mr. Brady with questions regarding the types of fluid stored in the frac tanks in the tank batteries on the southern and northern side of the pad, as to where the fluid in the tanks was transferred to when no longer intended to be used at the Ricci pad, whether or not the production fluid equipment was operational during the inspection and where the material in the vacuum box tanks will be disposed of. The questions I proposed to Mr. Roszel are in reference to the inspection conducted at the site on 2/12/18. Mr. Roszel responded to me via email on 2/20/18 and 2/22/18 providing answers to my questions. He stated that there is flowback fluid in the frac tanks on the fill slope (southern) side of the pad and BKV is keeping them pumped down, the frac tanks on the cut slope (northern) side of the pad contained fresh water and production fluid during the frac, but are empty and can be used for flowback if need be, the fluid in the f
2709612 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. There was water 4.5' below grade in the cellar and there was no bubbling noticed. The (9x5) annulus vent pipe was shut in and set up with 600psi pressure gauge with 70psi on it. There were empty 500bbl water tanks still on containment from a recent stimulation of the wells on site, the containment was mostly snow covered but what wasn't appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2776051 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted I performed an E&S inspection of the Ricci well pad. The pad recently had activity on site. There was still a few areas of containment erected and some apparatus on site.

The well pad and access road were solid and stable. The cut and fill slopes were well vegetated and did not appear to have any erosional issues.

The wells were properly labeled.

There were no apparent erosional issues.

There were no violations.

2814000 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
