This inspection is related to a nearby private water supply complaint, complaint ID 302271. Department records indicate that only one well has been drilled and stimulated at this time, the Bienko 4H (115-20969). Other wells that have been permitted for this pad are: 1H (115-21414), 2H (115-21415), 3H (115-21416), 5H (115-21417), and 6H (115-21418).
2353525 Primary Facility 2015-03-17 Drilling/Alteration No Violations Noted I visited the Bienko 2H on March 17th 2015. Falcon 27 was drilling the intermediate hole at the time of inspection, expected to cement intermediate casing later today. The surface string went well. There were no significant gas or water shows during drilling the surface string. Cement crew pumped 50 barrels of gel and 131 barrels of cement at 15.85 ppg, thixotropic blend. 34 barrels of good cement circulated to surface. Plug bumped, pressure on floats for 5 minutes, floats held, head left on 8 hrs, casing not disturbed during set time

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water and ice above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with about 10psi on it, the valve going to the production tank was closed. There was 713psi on the production casing and 369psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:05p.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. There was water above the (20x13) and (13x9) annuli and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2767802 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
