2291445 Primary Facility 2014-07-31 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 12:25 in response to a spill of (2) barrels of flowback fluid contained on the well pad. I was accompanied by Matt Shope, DEP O&G WQS during the inspection. Steve Jorgenson, site Company Man of Talisman Energy, who was on the location when the spill occurred, contacted the Department at 8:55 AM on 7/31/14 to report the spill incident. Mr. Jorgenson reported that the spill occurred at 3:55 AM on 7/3/1/14.
2321232 Primary Facility 2014-10-27 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 12:25 in response to a spill of (4) gallons of diesel fuel. There was no activity at the site during the inspection. I was accompanied by Frank Hoehle, DEP O&G WQS, during the inspection. Upon arriving to the location we observed (2) overpack drums, (1) (95) gallons in capacity, the other (65) gallons in capacity. We suspected that the drums contained impacted pad surface material associated with the diesel fuel release. We walked the pad surface area in and effort to locate the area where the release occurred. We noticed a small area of dark staining of the pad surface in the eastern section of the Pad at N-41°57’14” W-76°01" 13”. The pad surface material had a petroleum/diesel fuel odor to it. Based on our observations and the odor of the pad surface material where the staining was observed, we concluded that the diesel fuel released to the pad surface had not been compleltely remediated.

2498591 Primary Facility 2016-07-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m., the cellar was full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected and peaked at 58% LEL and 3% methane. There was 901psi on the production casing and 630psi on the production tubing. I took a walk around the location and noticed that the man hole on one of the production water tanks was leaking. I contacted Talisman Energy and spoke with Lance Rydle, I told him of the man hole cover and he told me that he would get it taken care of. There was no production water outside of the containment.

I left the site at 13:35p.m.

2588481 Primary Facility 2017-04-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:30a.m.

The cellar was full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with no gas detected. There was 647psi on the production casing and 395psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:10p.m.

2695392 Primary Facility 2018-02-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was about half full of water and ice with no bubbling noticed. The (20x13), (13x9) and (9x5) annuli were venting into the atmosphere and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2740459 Primary Facility 2018-06-08 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Traver pad. The wells associated with this pad are the 115-21429 and the 115-21430. These wells were properly labeled. The access road and pad surface were stable and solid. The vegetation surrounding the pad was lush and healthy and there were no erosional issues noted.

There were no violations noted.

2767472 Primary Facility 2018-08-23 Routine/Complete Inspection No Violations Noted I performed a Routine Complete Inspection of the Traver well pad. The wells associated with this pad are the 115-21429 and the 115-21430. The wells were properly labeled.

The township and state roads in the area, as well as all the creeks and waters had recently been hit hard by very heavy, sustained rainfall and many roads had been closed or under repair. The well pad access road and pad surface was unaffected however and there was no erosion noted.

The containment surrounding the well pad structures had some water in it but it was not excessive. There had been local rainfall as recently as last night. The containment all looked competent.

The well cellars had water but there was no bubbling present.

The cut and fill slopes as well as the topsoil pile all had adequate vegetation. There was healthy, perennial growth on all vegetated surfaces as well as some woody plant growth. No erosional forces appeared to be affecting the slopes at the time of inspection.

There were no erosional issues present

2793523 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection for the Traver Wellpad. The last company E&S inspection was completed in 10/21/2018. The wellpad was approximately 191355 square feet in size. It was 38 degrees Farenheit and overcast at the time of inspection.
    The pad was solid and stable.  There was no cut slope, but the pad was raised above the surrounding area.  On the slope leading in there was a fine crop of perennials.  The entire pad area was well established with perennial grasses, exceeding the 70% vegetative cover required by the Department.  The only areas where this was not the case was where the ground was simply too rocky to permit growth.  
    The temporary BMPs were all removed and the PCSM was installed as per the plan.
    The well was properly labeled.
    There were no violations noted.
2794702 Primary Facility 2018-10-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1.5' below grade in the cellar and there was no bubbling noticed. There was a methanol tank and pump (see photo) in containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
