2472031 Primary Facility 2016-03-16 Routine/Partial Inspection No Violations Noted The Department was on site to conduct a Routine Partial Inspection of the Butler Well Site located in Apolacon Township, Susquehanna County. At the time of the inspection, it appeared as though the well site met temporary stabilization requirements. Steve Haight signed off for an inspection on 3/15/16. There was a small area of stained rock on the access road close to the well pad. The Department requests that the operator investigate this issue, within 2 days of receipt of this report, and contact the Department with your findings.
2497414 Primary Facility 2016-06-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50p.m., a crew was working on the production units so the wells were shut in. The cellar was about half full of water with no bubbling noticed, there was cement between the (20x13) and (13x9) annuli’s with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 26% methane. There was 845psi on the production casing and 850psi on the production tubing.

I left the site at 13:30p.m.

The cellar was about half full of water with no bubbling noticed. There was cement visible at the (20x13) annulus and the (13x9) annulus. The (9x5) annulus was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 4% methane. There was 604psi on the production casing and 368psi on the production line. FPS Logistics LLC was pumping nitrogen down the production casing on the 1H to get fluid out of the well. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:35p.m.

2697768 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was cement between the (20x)13 and (13x9) annuli and there was no gas detected. There was a 500bbl tank connected to the 1H that was on containment. The containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2792247 Primary Facility 2018-10-04 Routine/Complete Inspection No Violations Noted I arrived at the site at 13:26. The weather conditions are cloudy skies and patchy fog, with light to moderate winds, heavy rain falling and, temperatures in the mid 60's. The rain had stopped shortly after I arrived, but resumed midway through the inspection. There was no activity or personnel at the site during the inspection. The Shea 1H and 2H wells are in production. I departed from the site at 13:59.
2793360 Primary Facility 2018-10-23 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1.5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
