2214668 Primary Facility 2013-10-17 Routine/Complete Inspection No Violations Noted The Department was on site at the Cabot Daniels Well Site to conduct a Preoperational Inspection. The site had formerly been a SWN site and does have a vertical well drilled which also has been transferred over to Cabot and is called the Daniels 1V (115-20541). Cabot has a second well permit issued for this pad, the Daniels E 3 (115-21496). The pad has been expanded from its original configuration to include a tank pad, hence the need for a preoperational inspection. The well pad is 90% complete, and the tank pad is 60% complete. The well pad is located in an exceptional value water shed, an UNT to Tunkhannock Creek. Wash rack is in place. A water truck is on site. All perimeter BMP’s in place. No violations noted.

The E&S Inspection Checklist indicated that Stephen Opeka conducted the most recent inspection on 12/9/16.

When the Department arrived on site it was noted that it was covered in a significant amount of snow. The Department was unable to verify if the Daniels Well Site met stabilization requirements. The Departments dug two smalls areas to the ground and determined that those areas appeared to meet stabilization requirements, therefore the Department plans to return after the snow melts to conduct a Follow-Up Inspection.

2559672 Primary Facility 2017-02-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:35a.m., the cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 654psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:30a.m.

2651520 Primary Facility 2017-10-24 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were connected together and going to the same tank. There was no bubbling noticed in the cellar. I took a walk around the location and there were no obvious concerns noticed.

2651774 Primary Facility 2017-10-23 Site Restoration No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permanent stabilization. The Department's determination is that the site meets the requirements for permit termination.

The well was in production. There was water 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
