The highest reading in the vicinity of the spill was approximately 1.48 mS/cm, and most readings were >1mS/cm.

There were no vegetative or erosional issues at the site.

2502199 Primary Facility 2016-07-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:50a.m., the cellar had water at the bottom with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with no gas detected, there was 590psi on the production casing and 393psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:10a.m.

2593804 Primary Facility 2017-05-11 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:15a.m.

The cellar was about a quarter full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with no gas detected. There was 490psi on the production casing and 393psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:40a.m.

The well was in production. There was snow and ice in the cellar 3' below grade and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2800004 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli were venting to the atmosphere. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
