2467817 Primary Facility 2016-03-21 Routine/Complete Inspection No Violations Noted On 3/21/16, the Department conducted an inspection of the SWN RU 29 Zeffer Pad and the 2 impoundments. The small impoundment was inspected first. The new liner has been installed. Normal operations are now occurring. During the inspection 2 water trucks were observed off-loading to this impoundment. All equipment associated with the dewatering and solids removal of the small impoundment has been removed. As suggested by the Department, the soil has been removed and replaced from the area between the off-loading concrete pad and the small impoundment. The disturbed areas has been seeded and mulched. The small impoundment appears to be about 1/3 full and the large impoundment appears to be approximately 1/3 full. All of the slopes associated with the pad and impoundments were inspected and appeared stable and well vegetated. No violations noted.

2509171 Primary Facility 2016-08-03 Routine/Complete Inspection No Violations Noted On 8/3/16, the Department conducted an inspection of the SWN Zeffer Well Pad and Impoundment’s 1 and 2. Also present during the inspection were Eric Rooney, Department Geologist and Mike Deming, SWN Senior Regulatory Analyst. Mr. Rooney was on-site evaluating SWN’s efforts in meeting their Work Plan submitted by CB&I consulting dated March 10, 2016, titled in part as “Additional Hydrogeologic Investigation Report for the Zeffer Impoundments.” At the time of the inspection, a small drill rig was on-site collecting core samples next to Monitoring Well # 8. This activity is being conducted to ensure that the monitoring well was constructed and installed properly based on the localized geology. A review of the large and small impoundment found that they were below the 2’ free board level required of waste water impoundments. The large impoundment had a free board level of about 4’, while the small impoundment was in the range of 5’. A review of the well pad found that there is good vegetative stability, a
2528649 Primary Facility 2016-10-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., met with company men Arnando Trujillo and Jonathan Sites. USWS (US Well Services) was on site to frack the Cramer 2H, 3H, 4H, 7H, and the Zeffer 4H wells. Upon arrival, Renegade Services was running a plug and perforation into the Zeffer 4H. The Cramer 2H, 3H, 4H, and 7H were completed and they just finished stage 4 out of 20 on the Zeffer 4H. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:45p.m.

2556170 Primary Facility 2017-01-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., the well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 806psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:25p.m.

2647439 Primary Facility 2017-10-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2669398 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection (RTNC) of SWN Well Pad 30 (Cramer) on Monday December 11, 2017. The wells which were on site were as follows:

115-21629 Zeffer N 4h 115-20559 Cramer 6h 115-20675 Cramer 5h 115-21096 Cramer 2h 115-21097 Cramer 3h 115-21099 Cramer 7h 115-21098 Cramer 4h.

The cut and fill slopes were not large and were well vegetated. The pad surface was solid and stable with no evidence of mud being tracked off site. The access road was solid as well. The wells were all clearly labeled and the Cramer 2h was sporting a cage around it. The production water liner was deteriorating but appeared to be watertight. It would be recommended this be checked and repaired asap. There were no violations

2710891 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues identified. No violations noted.
2752088 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted At time of inspection.
