Contractors are installing newly designed steel rectangular cellars. The cellars, when complete, will be welded to the conductor pipe creating a tight seal. This NOV will be associated with the 1H well permit.

2272011 Primary Facility 2014-04-17 Routine/Complete Inspection No Violations Noted The Department was on site on 4/17/14 at the Cabot Slocum S. Well Site to conduct an inspection. Inspection is a follow-up inspection to evaluate how the pad construction is progressing. Well permits that have been issued for this pad are the 1H (115-21666), 3H (115-21667), and the 5H (115-21668). The tank pad is approximately 95% complete. The perimeter BMP’s are in place for this pad. The top soil stock pile and a timber pile that have been installed above the cut slope of the tank pad have not been red lined onto the site plans. The well pad has been cleared and grubbed. Large pools of standing water are present on the well pad location. The site has still not evolved to the point where a pre-operational inspection would be warranted. This inspection does not qualify as a pre-operational inspection. Will re-inspect in near future
2272017 Primary Facility 2014-04-10 Routine/Complete Inspection No Violations Noted The Department was on site on 4/10/14 at the Cabot Slocum S. Well Site to conduct an inspection. Inspection based on Cabot notification that well spudding of this pad is to occur in the very near future, but that site conditions exist which are impeding construction. Well permits that have been issued for this pad are the 1H (115-21666), 3H (115-21667), and the 5H (115-21668). The tank pad is approximately 90% complete. The perimeter BMP’s are in place for this pad. A top soil stock pile and a timber pile have been installed on above the cut slope of the tank pad. Pointed out to George Bishop, GDS foreman, that these features were not on the E&S plans and that they needed to be red lined onto the site plans. 2273693 Primary Facility 2014-05-21 Follow-up Inspection No Violations Noted The Department was on site on 5//14 at the Cabot Slocum S. Well Site to conduct a follow up inspection. Well permits that have been issued for this pad are the 1H (115-21666), 3H (115-21667), and the 5H (115-21668).
2285945 Primary Facility 2014-05-21 Drilling/Alteration No Violations Noted H&P Drilling Rig #371 is the active rig on location, currently drilling the S Slocum 5H well. The current drilling depth is (4,743.21) feet. Cabot is drilling the lateral section of the well below the intermediate string through the process of water based mud drilling. A white powdery susbstance, was observed spilled to the drilling rig containment. A light tower did not have properly secured containment. The secondary containment berm in the backyard next to the drill cuttings containers was not secured properly. Mr. Wyant contacted rig hands just before the Department left the site to request the white powdery substance spilled to containment be cleaned up immediately and all insecure areas of secondary containment on the location be compleltely secured immediately as well.

2484895 Primary Facility 2016-05-17 Routine/Complete Inspection No Violations Noted On 5/17/16, the Department conducted an inspection of the COG Slocum, S. Pad located in Jackson Township, Susquehanna County. The inspection was in response to Cabot’s self-reporting of a spill of 12 gallons of antifreeze on the access road of the pad. According to Steve Watson, COG EH&S Specialist at around 7:00 PM, 5/16/16, the previous evening the driver of a low boy who was delivering a piece of machinery noticed that his truck was leaking antifreeze, a line had sprung a leak. J.P. Reilly is the contractor and began the clean-up of the spill. Not sure exactly how much material spilled on the access road as opposed to how much was lost in travel, but after the line was fixed, 12 gallons of antifreeze was added to the reservoir, hence the 12 gallon reporting. Today’s inspection revealed that approximately 3 tons of material has been excavated and placed in a lined roll-off container. According to Jose Fernandez of J.P. 2563597 Primary Facility 2017-02-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:45p.m., the cellar was about a quarter full of ice. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 624psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:15p.m.

The cellar had snow 5' below grade and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
