2467975 Primary Facility 2016-03-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15 a.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 564psi on the production casing. I took a walk around the location and noticed some spots on the lower side of the pad that looked like the slope was repaired. I spoke with Tioga Environmental Consultant employee Dillon Otten who was on site to conduct an E&S inspection. He told me that the slope was repaired and there trying to get the vegetation to take.

I left the site at 12:00 p.m.

2474549 Primary Facility 2016-03-30 Routine/Partial Inspection No Violations Noted The Department was on site at the Squire, R Pad to conduct a Routine Partial Inspection. David Schoonover (DEP-OGI), Jon Sassi (DEP-EPS), and I walked the site and noted that areas on the fill slope do not appear to be temporarily stabilized. Slope drains are present as well. The Department requests that the operator review the noted areas and deploy a method of erosion control that is consistent with your E&S plan.
2637587 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Arrived on location at 11:30 Weather: Sunny, warm. Approx. 64 degs.

A total of (2) wells have been drilled on this pad, the Squire, R. 2H and 3H wells. Both wells were in production at time of inspection. Both wells (13x9) and (9x5) annuli were open and venting, plumbed to common vent tank. Gas check performed at vent tank showed 100% LEL, 100% CH4. The Dept. requests that Cabot separates these wells (13x9) annuli from common vent line, leave open and venting to atmosphere for approx. 24 hrs., prior to performing follow-up gas checks. Please contact me to make arrangements to perform gas re-checks at these annuli. (Kenkennedy@pa.gov) Both wells were producing through 5 1/2" prod. casing. Prod. casing pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". The TFCU was password protected. Could not retrieve pressures or flow rates at time of inspection.

2638254 Primary Facility 2017-09-21 Follow-up Inspection No Violations Noted Arrived on location at 08:50. Weather: Foggy, warm. Approx. 64 degs.

On site in follow-up to inspection of 09/19/17. Met with Cabot field tech on site. As requested, both wells (13X9) annuli have been isolated and have been open and venting to atmosphere since ~ 08:40 today through 3/8" needle valve vent. Initial gas screening of (13x9) annuli were performed at 09:00, using a Altair 5X gas meter showed: -2H: Peak 28% LEL diminishing to 4% LEL with 0% CH4 detected. -3H: Peak 20% LEL diminishing to 4% LEL with 0% CH4 detected. Vents were left open and venting for an additional 15 min. Gas rechecks performed at 09:20 showed: -2H: Peak 18% LEL diminishing to 0% LEL with 0% CH4 detected. -3H: Peak 8% LEL diminishing to 0% LEL with 0% CH4 detected. Following gas screenings of annuli, a balloon flow check was performed at both wells (13x9) annuli. No flows were detected at either wells (13x9) annuli. Both wells were in production and (9x5) annuli remained open and venting to onsite vent tank during the

2692542 Primary Facility 2018-02-08 Routine/Complete Inspection No Violations Noted On location in response to Complaint #332211 received by Dept. on 2/6/18. Weather: Clear, cold, ~ 20 degs., approx. 6" of snow on ground. (2) wells were on this pad, the Squire, R. 2H and 3H wells. Both wells were in production at time of inspection. Both wells (13x9) and (9x5) annuli were open and venting, plumbed to common vent tank. Gas check performed at vent tank showed: 100% LEL, 100% CH4. Insp. performed on 9/21/17 confirmed that gas detected at vent tank is originating from both wells (9x5) annuli. A gas check performed of ambient air ~ 3' beside vent tank showed: 0% LEL, 0% CH4. Prod. casing, Static and Differential pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". The TFCU was password protected. At ~ 12:15 Cabot's well tender, Mitch, arrived on location. Mitch was at site to bleed well head press. off the 3H well, to add a "soap stick" to enhance production. Mitch explained that the 3H produces significant water and needs "soaped" daily and once or
2792890 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:49. The weather conditions are cloudy skies, cool, with light winds and temperatures around 60 degrees Fahrenheit. H&P Flex Rig #259 is on the location. The current drilling depth of the Squier 5H well is 16,860 feet. I met with James Cheeks, site company man, consultant of Cabot. Mr. Cheeks stated the following information in reference to site activities. "The Squier 5H well was drilled to total depth yesterday. Production casing was run at 6 AM this morning, 10/3/18. The next will to be drilled to total depth will be the Squier 1H. The top hole section of this well was completed , with the surface and intermediate section cemented by Precision Drilling Rig #181. The 7H and 5H wells have been drilled to total depth. The 10 H, 4H, 6H and 8H wells will also be drilled to total depth. Currently, the intermediate section of these wells has been, drilled, cased and cemented. The 2 existing wells, the Squier 2H and 3H, are currently shut in due to drilling activities". I depar
