2337095 Primary Facility 2015-01-14 Routine/Complete Inspection No Violations Noted On 1/14/15, the Department was on-site at the Cabot Friedland Farms Well Site and Tank Farm to conduct an inspection of the E&S controls. Well permits that have been issued for this site are the 1H (115-21677), 3H (115-21678), 4H (115-21679), 5H (115-21680), 6H (115-21681), 7H (115-21682), 8H (115-21683), 9H (115-21684). The Precision Rig 569 is on-site and is, according to a recent notification, in the process of drilling the 7H. A review of the perimeter revealed that the site stability looked good. Rig containment and layout looked adequate and organized. No violations noted.

2401298 Primary Facility 2015-08-13 Routine/Complete Inspection No Violations Noted As the Department arrived at the site at 11:00, Cabot was in the process of setting up production equipment and conducting treatment of hydraulic fracturing flowback fluid.
2448651 Primary Facility 2016-02-02 Routine/Complete Inspection No Violations Noted Cabot was in the process of conducting on-site pretreatment of hydraulic fracturing flowback fluid and production fluid generated from the Friedland Farms wells on the tank pad. I recorded (2-3) ms/cm³ conductivity in water on containment near a tank storing freshwater and on the ground surface in between the containment berm and an above ground pipeline. Sodium sulfate, a chemical used in the treatment process, had been spilled to containment near a pump and the freshwater tank. Mark Banyas of Comtech informed me that wind gusts had dispersed sodium sulfate out of open bags stored on a tank used in the treatment process onto containment. Calcium chloride has been used as a deicing agent at the site according to Mr. Banyas. A whitish precipitate/residue was observed on containment behind tanks used in the treatment process. Mr. Banyas stated the water with the elevated conductivity will be vacuumed up and placed into the influent tanks and the containment where the white staining was observed will be
2467674 Primary Facility 2016-03-16 Follow-up Inspection No Violations Noted Shortly upon arrival at the site, we observed wet soil and seeps on the surface of the fill slope on the east side of the tank pad below the tank battery. The majority of the water is located at the bottom of the fill slope, near compost filter sock and the limit of disturbance of the site. There is also an area of pooled water just below the top of the fill slope and tank battery containment berm. The tank battery containment had water accumulated on it in certain areas due to recent rainfall events. There is also water pooled on the ground surface, behind the tank battery next to the containment berm, in the north section of the tank pad. The conductivity was measured in the range of (3-5) milliSiemens/cm³ in water on the fill slope, (1-2) milliSiemens/cm³ in water on the ground surface behind the tank battery, and less than (200) microSiemens/cm³ in water on the tank battery secondary containment.
2472994 Primary Facility 2016-03-22 Follow-up Inspection No Violations Noted There was no activity on the tank pad. Shortly upon our arrival, Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas, arrived at the site. The intent of the inspection was to re-screen the seeps on the fill slope, water in the tank battery containment and tank pad surface for conductivity. Water in these areas of the tank pad was screened for conductivity during the Department's 3/16/2016 inspection. There were a total of (16) screening points conducted on the tank pad. I used a Hanna Soil Test and Electrical Conductivity & Temperature meter, an Oakton Multi-Parameter Tester 35 meter, and EXTECH Instruments ExStik 2 meter to field screen water on the tank pad fill slope, ground surface, and containment for specific conductance (conductivity). Mr. Watson used a Hanna HI928 PH 7 meter. He filled the water in a small bottle in order to get a reading.
2473756 Primary Facility 2016-04-04 Administrative/File Review No Violations Noted Andy Mehalko, Environmental Supervisor, of Cabot Oil and Gas contacted the Department on 4/1/16 at 11:00 AM to report the discovery of a spill indicative of flowback or production fluid. Cabot had collected split water samples with the Department on 3/22/16 on the fill slope of the tank pad, the frac tank battery secondary containment liner and the ground surface of the tank pad due to elevated conductivity readings first discovered by the Department on 3/16/16. The fill slope is located on the east side of the tank pad. The samples of fluid water accumulated in the tank battery containment and tank pad surface near the water on containment were collected on the west side of the tank pad. Mr. Mehalko informed the Department of the following. "Sample results indicate flowback or production fluid has impacted the ground surface of the tank pad. Resource has been engaged and commencing the process of site characterization. They will install an interceptor trench along the east side of the location early i
2480828 Primary Facility 2016-05-02 Administrative/File Review No Violations Noted This administration inspection is created to document the Department's discovery of elevated electrical conductivity readings at the Friedland Farms Well Site. The elevated readings were found in seeps on the surface of the fill slope on the east side of the tank pad, below the frac tank battery staged on secondary containment, and in water accumulated on the ground surface in the north and west side of the tank pad as documented in the Department's 3/16/16, 3/22/16 and 4/4/16 inspection reports. Due to the elevated conductivity readings, the Department and Cabot had recorded in the seeps and the water on 3/16/16, a total of (3) samples were collected by the Department at the site. Sample number 4559-210 was collected in a seep on the fill slope of the tank pad. Sample number 4559-211 was collected in water on the ground surface on the west side of the tank pad. Sample number 4559-212 was collected in water on the tank battery containment liner near sample number 211. The sample results revealed eleva
2521650 Primary Facility 2016-09-13 Routine/Complete Inspection No Violations Noted On 9/13/16, the Department conducted an inspection of the Friedland Farms P1 Well Site. All eight wells associated with this pad appear to be in production. Approximately 30 frac tanks are being stored on-site. A review of the tank pad found 2 large soil piles placed on temporary containment. The soil piles are present as a result of pad material having been excavated in response to a spill that was documented in inspection ID # 2480790 dated 5/2/16. An inspection of the temporary containment, as shown in the 2 attached photographs, shows that the containment’s present condition for both piles is no longer adequate in functioning to keep storm water from directly contacting the possibly contaminated soil. Steve Watson, Cabot EH&S Specialist was contacted the following day and advised to correct the situation immediately.
2521660 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted On 9/22/16, the Department conducted a follow-up inspection of the Friedland Farms P1 Well Site in order to evaluate the present condition of the temporary containment still located at this location. Present during the inspection was I and Jon Sassi, Department Environmental Protection Specialist. Prior to arriving at the site, Steve Watson, Cabot EH&S Specialist was contacted by phone and stated that the small pile had been removed and had been disposed of at the Keystone Landfill. Steve stated that the large pile had recently been tested and found not to be contaminated, and that this material will be placed back into the trench that the material came out of. The trench was excavated and developed as part of the remediation work being done in response to the spill documented in the Jon Sassi 5/2/16 inspection. 2666947 Primary Facility 2017-12-04 Routine/Complete Inspection No Violations Noted Arrived on location at: 10:00. Weather: Clear, Cold, ~ 35 degs. A total of (8) wells were present on this pad, the Friedland Farms 1H, 3H, 4H, 5H, 6H, 7H, 8H and 9H. All (8) wells were in production at time of inspection. -All wells Prod. Line, Prod. casing, Static and Differential pressures were being monitored by wireless press. monitoring units sending pressures to an onsite Total Flow Control Unit "TFCU". The TFCU Control Panel was password protected, no pressures were accessible at time of inspection wireless sending units. All wells (13x9) and (9x5) annuli were open and venting, plumbed to a shared, common vent tank. These annuli were also equipped with wireless press. monitoring units. A Gas check performed at the shared, common annular vent tank showed 100% LEL, 20% CH4. - Overall, wells and pad were found in good condition.

See attached pictures for further info. No obvious concerns were noted at time of inspection. No violations were noted. Left location at 11:30.

2748511 Primary Facility 2018-06-15 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1315 hrs to conduct a RTNC inspection. Present to represent the Department were Gene Rickard (WQS-DEP) and myself.

The access road, pad surface, and all berms/slopes were solid and stable. Required BMPs were present and in good condition, with the exception of a deteriorating filter sock at toe of fill slope. (Photo 6) Please review this filter sock and determine if it should be repaired or removed.

The well heads were clearly and properly labeled with names and permit numbers. Required signage was present at entrance to access road. All required paperwork was present in the mailbox.

The Department concluded its investigation and left the site at 1350 hrs. No violations were noted.

2819049 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All wells appear to be in production. No E&S issues identified. Good vegetative stabilization present. No violations present.
