Sodium sulfate, a chemical used in the treatment process, had been spilled to containment near a pump and the freshwater tank. Mark Banyas of Comtech informed me that wind gusts had dispersed sodium sulfate out of open bags stored on a tank used in the treatment process onto containment. The majority of the water is located at the bottom of the fill slope, near compost filter sock and the limit of disturbance of the site. There is also an area of pooled water just below the top of the fill slope and tank battery containment berm. The tank battery containment had water accumulated on it in certain areas due to recent rainfall events. There is also water pooled on the ground surface, behind the tank battery next to the containment berm, in the north section of the tank pad. The conductivity was measured in the range of (3-5) milliSiemens/cm³ in water on the fill slope, (1-2) milliSiemens/cm³ in water on the ground surface behind the tank battery, and less than (200) microSiemens/cm³ in water on the tank battery secondary containment. |

Shortly upon our arrival, Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas, arrived at the site. The intent of the inspection was to re-screen the seeps on the fill slope, water in the tank battery containment and tank pad surface for conductivity. Water in these areas of the tank pad was screened for conductivity during the Department's 3/16/2016 inspection. There were a total of (16) screening points conducted on the tank pad. I used a Hanna Soil Test and Electrical Conductivity & Temperature meter, an Oakton Multi-Parameter Tester 35 meter, and EXTECH Instruments ExStik 2 meter to field screen water on the tank pad fill slope, ground surface, and containment for specific conductance (conductivity). Mr. Watson used a Hanna HI928 PH 7 meter. He filled the water in a small bottle in order to get a reading. |

Cabot had collected split water samples with the Department on 3/22/16 on the fill slope of the tank pad, the frac tank battery secondary containment liner and the ground surface of the tank pad due to elevated conductivity readings first discovered by the Department on 3/16/16. The samples of fluid water accumulated in the tank battery containment and tank pad surface near the water on containment were collected on the west side of the tank pad. All eight wells associated with this pad appear to be in production. Approximately 30 frac tanks are being stored on-site. A review of the tank pad found 2 large soil piles placed on temporary containment. The soil piles are present as a result of pad material having been excavated in response to a spill that was documented in inspection ID # 2480790 dated 5/2/16. An inspection of the temporary containment, as shown in the 2 attached photographs, shows that the containment’s present condition for both piles is no longer adequate in functioning to keep storm water from directly contacting the possibly contaminated soil. Steve Watson, Cabot EH&S Specialist was contacted the following day and advised to correct the situation immediately. |

Prior to arriving at the site, Steve Watson, Cabot EH&S Specialist was contacted by phone and stated that the small pile had been removed and had been disposed of at the Keystone Landfill. Steve stated that the large pile had recently been tested and found not to be contaminated, and that this material will be placed back into the trench that the material came out of. The trench was excavated and developed as part of the remediation work being done in response to the spill documented in the Jon Sassi 5/2/16 inspection. Line, Prod. casing, Static and Differential pressures were being monitored by wireless press. monitoring units sending pressures to an onsite Total Flow Control Unit "TFCU". The TFCU Control Panel was password protected, no pressures were accessible at time of inspection wireless sending units. All wells (13x9) and (9x5) annuli were open and venting, plumbed to a shared, common vent tank. These annuli were also equipped with wireless press. monitoring units. A Gas check performed at the shared, common annular vent tank showed 100% LEL, 20% CH4. - Overall, wells and pad were found in good condition.

The access road, pad surface, and all berms/slopes were solid and stable. Required BMPs were present and in good condition, with the exception of a deteriorating filter sock at toe of fill slope.
