Well Site and Tank Farm to conduct a Preoperational Inspection. The well permits that have been issued for this pad are the Lerner E 1H (115-21891), 2H (115-21892), 3H (115-21893), 4H (115-21894), 5H (115-21895), and the 6H (115-21896). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. 2 Spudder Rigs on-site prepping to drill. No violations noted.
2342846 Primary Facility 2015-01-14 Routine/Complete Inspection No Violations Noted Orion Drilling Rig #16 is the active drilling rig on the location currently drilling the lateral (horizontal) section of the G Lerner 5 H well at a depth of (12,187.89) feet with synthetic based drilling mud. I observed what appears to be a relatively small amount of oil based mud spilled to containment along with drill cuttings overflowing a container storing the cuttings onto containment in the backyard. Mr. Hornsby stated that a truck is on way to remove drill cuttings in the containers and on containment. I also observed a relatively small area of dark staining on the pad surface next to a frac tank near the diesel fuel tank.
2364794 Primary Facility 2015-04-24 Routine/Complete Inspection No Violations Noted On 4/24/15, the Department conducted an inspection of the well connect pipeline tie in occurring for the COG Lerner E. Well Pad. During the inspection, the Department identified violations related to the Well Pad. The Department observed that all of the well pad fill slopes were experiencing significant slope failures. Approximately 5% of the cut slopes were experiencing significant slope failures. Please address immediately. Enforcement record # 322504 is tied to inspection record # 2364703, the well pad site.

2379070 Primary Facility 2015-05-12 Routine/Complete Inspection No Violations Noted On 5/12/15, the Department conducted a follow up inspection of the COG Lerner E. Well Pad to assess the repairs conducted by Cabot to stabilize the pad slopes. As of this inspection, all violations noted in the inspection conducted on 4/24/15, inspection record # 2364703 and associated enforcement record # 322504 have been corrected. Approximately 90% of the failed slopes have been stabilized via rock stabilization. Attached to the inspection is a copy of Cabot’s written response to the 4/24/15 inspection. No further action is required of Cabot at this time by the Department.
2397726 Primary Facility 2015-07-28 Routine/Complete Inspection No Violations Noted Shortly upon arrival while in the process of surveying the production equipment and pad surface in the general area where this equipment is staged, I noticed small spots of dark staining in the southeast corner of the pad surface at N- 41° 47' 44”; 75° 33'17" near an Erick Flowback Services Gas Buster Tank. I used a Hanna Soil Test Electrical Conductivity & Temperature meter to screen the spots for soil conductivity. The highest conductivity reading I recorded was (2.9) milliSiemens/cm³. On average conductivity readings were in the range of (0.5-1) milliSiemens/cm³. I observed a white powdery substance spilled to the pad surface near where the treatment process equipment is staged. The material may be a chemical utilized in the process. GWTT indicated they would clean up this material spilled to the pad surface. Mr. Watson used yellow spray paint to mark off the area of the pad surface where the dark staining was discovered and indicated it would be remediated.

The well was in production. The cellar was empty and there was no gas detected. The (20x13) annulus was open to atmosphere and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. There was water 5.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2798577 Primary Facility 2018-11-02 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site does not meet the requirements for permit termination. The Post Construction Stormwater Management Plan provided with this NOT application does not reflect The as-built conditions. Primarily, the Stabilized Channel With S75 Matting does not exist. A buried cross culvert has been installed in its place. Please provide a revised Post Construction Stormwater Management Plan which accurately reflects site conditions with the next NOT submittal for this site. I recommend the NOT not be approved.
