2344126 Primary Facility 2014-02-13 Preoperation Inspection No Violations Noted At the time of the inspection, the Site ID Sign was installed and the E&S Plan was on site. The E&S Inspection Log indicated that Dillon Otten (TEC) conducted a weekly inspection on 2/12/15. It was noted that the site is snow covered. Mr. Christian stated that they installed erosion control blanket over the slopes. The perimeter BMP’s have been installed around the well site and the tank pad. The inspection focused only on the well site as the tank pad was currently under construction. It appeared as though the BMP’s were installed in a manner that is reflective of the E&S Plan.
2599877 Primary Facility 2017-05-16 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. Upon my arrival I reviewed the E&S Inspection Log to find that the most recent inspection has been conducted on 5/15/17 by Sam Wiernuscz and it was a post precipitation inspection.

During the course of the Department's inspection, I conducted a visual inspection of the wells. I found bubbles forming at the unions on the bull plug, with a gauge, on the production tree of the 2H and 6H wells.

2627456 Primary Facility 2017-08-21 Follow-up Inspection No Violations Noted I received an email from Mike Pirner of Cabot letting me know most repairs to the Wright well site were now completed. Upon inspection of the site I found that they were indeed completed and stability had been restored to the site previously affected by torrential rains. The entire area was now reseeded, limed, fertilized and mulched. The slopes were starting to green up where erosion blanket had been placed and the sediment basins were reshaped, reseeded and mulched.

There were no violations noted.
