2531966 Primary Facility 2016-10-26 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:45p.m. for a new spud inspection, Minichi Contracting Group was on site to drill the holes and run the conductor pipe. I spoke with Keith Bowman of Minichi, the rig was drilling the hole for the 1H and they were at a depth of 31' and plan on drilling to 88'. When the 1H is completed, they plan on drilling the 3H and setting the conductor pipe at a depth of 88'. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. Halliburton had trucks staged on the well site. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:05p.m.

2537807 Primary Facility 2016-11-14 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:05p.m., met with company man Tony Anthony, Falcon Rig 23 was on site and drilling the top hole section of the well to kick off. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 4056'. Projected TD for the top hole section is 5515'. I left the site at 13:50p.m.

2553406 Primary Facility 2017-01-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:30a.m., met with company man John Woodburn, SDC Rig 45 was on site and drilling the lateral section of the well. The top hole section was drilled and completed on 11/15/16. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 5794'. Projected TD for the well is 13910'. I left the site at 10:15a.m.

2564537 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50p.m., I met with company man Jeff Voltz.

US Well Services was on site to frack the 1H and 3H wells. At time of arrival US Well Services was shut down and waiting on a part for repairs. I took a walk around the location and there were no obvious concerns noticed. The 1H was completed with 31 stages. They were preparing to start the 3H when they went down, once repairs are made they will resume with the frack. The 3H will have 34 stages. The water is being pumped in from the Zeffer production water impoundment and the Odel fresh water impoundment. I left the site at 14:10p.m.

2600913 Primary Facility 2017-06-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:50a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 770psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:25a.m.

2685316 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted Inspection conducted as a routine inspection. Site snow covered. Site is plowed. Well tender reviewed well heads during the inspection. No E&S violations observed. No spills detected. No violations noted.
2709564 Primary Facility 2018-03-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I was unable to check the cellar because there was containment around the well from recent service work. There was tubing recently put in the 1H and 3H wells. The containment was mostly covered with snow and the sections that were not appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2807719 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836133 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Overall the site has good stabilization. No E&S issues identified. No violations noted.
