Six wells (1H thru 6H) are drilled and in production and a seventh well (7H) is permitted. There are several slopes at this location that need further stabilization. There are several small pieces of liner on the pad surface and in the soil around the edges of the pad. This material should be disposed of properly. A puddle in the center of the pad exhibited an elevated reading for specific conductance. The pad surface and surrounding areas should be screened and a plan on what action will be taken should be communicated to the Department. The water impoundment on site has been drawn down and contains sediment and sediment laden water at this time. No violations were cited during this inspection. A follow-up inspection will be conducted to determine if these issues have been addressed in a timely manner. - MRN

During today’s inspection the area where the spill occurred had been excavated and the contaminated soil placed in a roll-off dumpster and covered with plastic. David Smith Jr., Production Superintendent for Ultra Resources, had stated in a phone message left the night of July 15, 2012 that soil samples had been collected and submitted for analysis. The violations cited on July 9, 2012 remain outstanding at this time; no new violations were cited during this inspection. - MRN

2186084 Primary Facility 2013-07-22 Routine/Partial Inspection No Violations Noted On July 23, 2013 an inspection was conducted at the Thomas 808 pad in Elk Township, Tioga County. The last inspection conducted by AMEC on behalf of the operator was on 7/17/2013 and no issues were noted. The E&S controls remain in place and are functioning. Several areas were observed that require further stabilization, though no sediment transport was observed during this inspection. Puddles on the pad surface and the water in the sediment trap were screened but none exhibited elevated conductance. No violations were cited during this inspection.

2549931 Primary Facility 2016-12-29 Routine/Partial Inspection No Violations Noted Routine partial inspection. Site was idle and snow covered. One production tank was observed staged in containment east of the separator units. The containment held snow and a 55-gallon drum had been placed inside. The original production tanks, which had been situated north of the current tank, have been removed. The original tank farm included four riveted tanks within a permanent containment structure. According to reclamation schedule provided by SWEPI, interim restoration of site was to be completed in June 2016. Vegetative cover and installation of post-construction stormwater management BMPs were not verified due to the snow cover. TLT
2625712 Primary Facility 2017-08-09 Site Restoration No Violations Noted On 8/9/17 at 1114 hrs I conducted an inspection of the Thomas 808 well site. No one was present during the inspection. It was mostly sunny and 71 degrees F.

I observed the site (Photos #1 & 2) and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the access road and well pad. Six wells, the Thomas 808 1H - 6H, have been drilled. According to DEP records, the 1H, 2H, 5H and 6H wells are in production. The 3H and 4H wells were placed on inactive status on 9/7/12.

Produced water is stored in a production tank, which is staged in secondary containment (Photo #3). Routine secondary containment inspections are being completed; the last inspection was on 8/5/17. Water in the containment structure had a field measured specific conductance reading of 5,160 micro Siemens/cm. Puddles on the pad were also tested; readings ranged between 860 - 1,490 micro Siemens/cm.

2627574 Primary Facility 2017-08-21 Administrative/File Review No Violations Noted On 8/21/17 I reviewed the Thomas 808 Well Site Restoration Report (WSRR). A revised report was submitted on 8/15/17. The original WSRR was submitted on 7/25/17 and included the Marshlands K Thomas Unit 1 well, which is located at a separate well site. The revised WSRR does not include the Unit 1 well.

Site conditions were verified during an inspection conducted on 8/9/17; the inspection report documents that impervious areas have been minimized, post construction stormwater management practices have been installed and the site is permanently stabilized.

The WSRR was acknowledged on 8/21/17. No violations are cited in association with this administrative inspection. TLT
