The 5/31/12 and 6/11/12 violations associated with the impoundment remain outstanding against the Phoenix 590934 well (117-21148). 2601420 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted I arrived on-site @ 09:35 hrs. and met Bill Wallace of EQT. Patterson UTI rig 294 on location tripping into the 593851 well. Some housekeeping issues noted, all in containment and mostly related to precipitation; Mr. Wallace informed me that a vac truck was en route. I noted 2 spots where the primary containment may have been allowing rainwater through to the pad; Mr. Wallace was aware of one of the spots, and will have both checked and/ or repaired. I departed the site @ 10:20 hrs.
2613911 Primary Facility 2017-07-06 Routine/Complete Inspection No Violations Noted I arrived on-site and met Bill Wallace, EQT company man. Patterson UTI rig #294 on location. Bill informed me that they had just started drilling on the 593853 well ( permit #117-21786). He also told me that after my last inspection in which I identified some spots where the pad containment appeared to be leaking that they had wound up moving the rig off the site, pulling up the rig mats, and replacing the pad-wide containment due to issues with the liner system. The new liner was in good shape and appeared to be a heavier, thicker material. The pad was in good condition overall, with no issues noted.
2650823 Primary Facility 2017-10-18 Routine/Partial Inspection No Violations Noted On 10/18/17 at 0837 hrs I conducted an inspection at the Phoenix S site. It was sunny and 41 degrees F. EQT OSR, Chip Miller, accompanied me during the well pad inspection.

I observed the well pad and water storage pad; the former Phoenix S Pit was not inspected. The Phoenix 593850-593855 wells were drilled Jan.-July 2017; stimulation notices were received in September. EQT was drilling out plugs on the 593851 well and flowing back the 593850 and 593852 wells. Well pad containment consists of bermed, pad-wide containment with mats on top. Localized containment structures were observed under individual pieces of equipment. Standing water in the pad-wide containment was screened in the following locations for specific conductance: SE pad corner: 672 microsiemens/cm NE corner: 10.16 mS/cm NW corner: 7.04 mS/cm SW corner: 920 microsiemens/cm A tank farm (500 bbl tanks) was staged along the north side of the pad and held produced water from flowing back the wells. 2660416 Primary Facility 2017-11-12 Incident- Response to Accident or Event No Violations Noted Rich Herbstritt (DEP, OGI) and I inspected this site after EQT reported loss of well control on one of the three Marcellus wells FTS (contractor) was hydraulically fracturing ("zipperfrack"). We spoke with EQT's VP of drilling/ completions on site, Jim Helmick, who explained the following: last night at ~2330 hrs, the Cactus (brandname) well head began to leak flowback and methane while FTS was fracturing. Between then and noon today, vacuum trucks have been removing the flowback liquid and delivering it to 3 locations: Hydrorecovery in Antrim, PA, a Seneca Resources site, and to an Above-ground Storage Tank (AST). They estimate vac trucks removed 1,900 barrels between 2330 and noon, and that the well is still releasing ~110 barrels/hr (~1 vac truck load/hr) into the cellar. Vac trucks have been able to keep up with removal of liquid from the cellar, preventing overflows. Wild Well Control out of Texas has personnel on site who are addressing the well control issue. 2679509 Primary Facility 2018-01-04 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the EQT Phoenix pad S located in Duncan Township, Tioga County on January 4, 2018 at 11:30 hrs. The weather was 17 degrees. Overcast with 3 inches of snow on the ground. 17 mph winds out of the NW. Humidity 67%. Pressure 29.7 inHg.

There was no activity onsite during the inspection. Pad wide containment and rig matting appears to still be in place beneath the snow cover. Two residual waste vac trucks are parked onsite. The previous inspection noted the Phoenix 593850-593855 wells were drilled Jan.-July 2017; stimulation notices were received in September. The previous inspection conducted in Oct. noted the wells were being drilled out and flowed back. No issues were noted on the pad.

The AST (Aboveground Storage Tank) located across the access road from pad S is labeled "impaired water only". Trucks were observed idling on containment. No visible staining or fluid was observed in the area adjacent to the AST.

2746004 Primary Facility 2018-06-19 Routine/Complete Inspection No Violations Noted The DEP inspected the EQT-Phoenix Pad S located in Duncan Twp, Tioga Co. on 6/19/2018 @1300 hours. The weather was 78 degrees, 45% humidity and winds N 12mph. Mike Cromley, EQT Env Coordinator and Stan Bridges, EQT Company Man were present during the inspection. The well site is currently set up for flowback.

I observed the well pad and water storage pad. The Phoenix 593850-593855 wells were drilled Jan.-July 2017; stimulation notices were received in April for the 593853-593855. As of today's inspection, EQT was completing flow back of the 59383-593855 wells.

2808174 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the EQT - Phoenix Pad S located in Duncan Twp, Tioga Co. on 11/28/2018 @1300 hours. The weather was cloudy/light snow 31 degrees, Humidity 71%, Winds WNW 19 mph. Two Beimel trucks were onsite emptying fluid from the production tanks onsite.

The Phoenix 593850-593855 wells were drilled Jan.-July 2017; stimulation notices were received in April for the 593853-593855. In June of 2018 EQT completed flow back of the 59383-593855 wells. As of today's inspection, the pad is setup for production.

No areas of elevated conductivity were located on the pad surface. Perimeter E&S controls appeared to be functioning. Violations against the Phoenix 590934 API#117-21148 remain OUTSTANDING.

2845421 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted I arrived onsite to meet with Doug Oberdorf of EQT. One EWS evaporator was present on the pad, but had not yet been hooked up to the production tanks or placed in containment. Gregg Johnson of EWW was also present. Gregg informed me that high winds on Monday had caused them to postpone placement of the evaporator unit. The OG71A for the site called for 3 evaporator units; Gregg informed me that the trial would begin with 1 unit, and the other 2 would be added in later. The production tank containment appeared to be in good condition, with less than 2" of ice. Several of the well cellars had water in them within 2' of the top; I recommend that these be monitored and vacced out before overtopping.
