Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2764545 Primary Facility 2018-08-15 Routine/Complete Inspection No Violations Noted This well is currently being drilled. Cuttings are being directed into an unlined pit. The well permit is clearly posted on a tree close to the access road. All areas that would have been disturbed during construction have been stabilized with stone or seed and straw. No violations noted at this time.
2795992 Primary Facility 2018-10-29 Routine/Complete Inspection No Violations Noted This location can most easily be accessed off of Bauer Hill Rd. in Glade Township, Warren PA. This access point is the one that was most recently worked on, and referenced in past reports. There are some ruts forming near the top of the road, that could use grading. This portion of road is holding up well otherwise. Some portions of the accessing road, beyond this area, have developed some rutting as well. Some grading and reshaping is recommended, pending completion of this latest well and location. This location can also be accessed from behind a storage facility, further up on Cobham Park Rd.

Frack pipe is currently being tripped from the well. The well was completed with ten fractured stages, according to the contractor on site. Water was being pumped from a small pond near the location. The area under the rig floor and the entire pit are covered by a synthetic liner. All flow back appears to be making its way to the pit. Free board appears to have been maintained. No unwanted spills or fluid migrations
