2755138 | Primary Facility | 2018-07-23 | Routine/Complete Inspection | No Violations Noted | This inspection report has been created to document today's meeting between the operator, Mr. Dan Anderson, the property owner, Mr. Mike Shanshala, and my self. The meeting was with regard to maintenance of a section of access road servicing the latest locations permitted to the operator. The operator has agreed to add cross pipe at 3 to 4 separate locations. He has also agreed to widen and add needed ditch-lines to the steepest and narrowest section of this road. Progress for this project will continue to be monitored in future inspections.
Location Permit #123-48176: This location has been built. The location is a bump out off of the existing access road. Sand stone has been added and graded to form the base. The well permit and E & S plan are posted on a nearby tree. No violations are noted at this time. |
2767132 | Primary Facility | 2018-08-23 | Routine/Complete Inspection | No Violations Noted | This facility is in the drilling stage of development. The drilling contractor is currently tripping drill string out of the newly established well bore. Drill cuttings were directed into an un-lined pit. The pit does appear to be holding some water. The well permit is clearly posted on a tree near the location.
Several improvements have been made to the access road for this well site. The steepest portion of the road has been widened, crowned and has had ditch line added to both sides. Several culverts have been added and commercial stone has been broadcast on the surface of the road. Seed and straw appears to have been broadcast over all excavated areas. No violations are noted at this time. |
2790020 | Primary Facility | 2018-10-16 | Drilling/Alteration | No Violations Noted | This location can most easily be accessed off of Bauer Hill Rd. in Glade Township, Warren PA. This access point is the one that was most recently worked on, where the road was straightened and culverts were added. The road appears to be holding up well. The location can also be accessed from behind a storage facility, further up on Cobham Park Rd.
The well is currently being fracked. According to the service rig contractor, they are currently on stage 11 of 13. Water is being pumped from a small pond near the location. The area under the rig floor and the entire pit are covered by a synthetic liner. All flow back appears to be making its way to the pit. The water appears to be getting recirculated and reused. No unwanted spills or fluid migrations have been observed. The permit is clearly posted on a tree near the location. |
