2525026 Primary Facility 2016-05-11 Follow-up Inspection No Violations Noted On 5/11/16, I performed a follow up inspection at the J Phillips 1 well site in response to complaint number 322744 and made the following observations:

•Well is located in what appears to be a hay field with no direct access road. •Small rings of dead grass were noted around each piece of well equipment, uniformity indicates herbicide spraying as part of well maintenance. No violation. •A roughly triangular area of excavation around six feet across and around four inches deep was noted near the production tank. Follow up inspection needed to test for contamination.
