Moved onto this well on 3/20/17 and resumed drilling at 40 ft. The 24” casing was set at 241 ft. on 3/21/17 and cemented to surface with 4 bbls returned. The 18 5/8” casing was set at 343 ft. on 3/23/17 and grouted to surface above a basket. Coal was found from 287 ft. to 293 ft. Two baskets were set at 275 ft. Currently at 390 ft. and waiting on cement.

The 13 3/8” casing was set at 1592 ft. on 3/25/17 and cemented to surface with 36 bbls returned. A Casing Annulus Packer was set from 1106 ft. to 1116 ft. in the 13 3/8” casing. TD was at 1640 ft. on 3/24/17.

2720118 Primary Facility 2018-04-19 Incident- Response to Accident or Event In Compliance with Policy On 4/19/18 an incident inspection of the Vivian Harbison Well Site was conducted in response to CTS ID: 333587. The following was observed:

>>On 4/13/18 EQT's Mike Cromley reported a ten gallon spill with cleanup beginning immediately as a vacc truck was present on site at the time of the spill.

>>Further communication with operator personnel indicated a spill of 10 gallons of produced water near the offloading point near the production tanks and that the spill cleanup was completed the same day.

>>Site inspection found no visible evidence of contamination, cleanup appears to have been fully completed in compliance with policy.
