Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2650221 Primary Facility 2017-10-20 Drilling/Alteration No Violations Noted Cellar Tech auger rig drilled a 30" well bore and set 26" conductor casing at 40 ft. on 10/16/17.
2654404 Primary Facility 2017-10-27 Drilling/Alteration No Violations Noted Fountain Quail Rig #8 moved onto this well on 10/20/17 and resumed drilling at 53 ft. KB. The 18 5/8" casing was set at 284 ft. on 10/21/17. Operator was unable to displace cement to surface. Tagged cement at 87 ft. and grouted casing to surface. The 13 3/8" casing was set 444 ft. on 10/22/17 and grouted to surface above a basket. Coal was found from 344 ft. to 352 ft. A basket was set at 300 ft. Currently at 494 ft. Stopped drilling and moved to next well on location.
2660204 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted On 11/10/17 a routine complete inspection of the R. Smith well site was conducted in response to notification additional drilling on site. The following was observed:

>>A box tank was found near the impoundment with no containment. Remediation recommended, no violation.

>>A new top soil stock pile was noted on site. It was stabilized with straw at the time of inspection.

>>The soil stock pile was noted to not have any functional controls below it other than an adequate vegetated strip. Decomposed CFS should be removed, repaired, or replaced. No violation.

>>Drilling was ongoing at the site at the time of inspection.

2661161 Primary Facility 2017-11-15 Drilling/Alteration No Violations Noted Fountain Quail Drilling Rig #8 reentered this well on 11/11/17 and resumed drilling at 494 ft. The 9 5/8" casing was set at 1677 ft. on 11/13/17 and cemented to surface with 26 bbls returned. A Casing Annulus Packer was set in the 9 5/8" casing string from 1267 ft. to 1277 ft. Currently at 2950 ft. and TIH after picking up directional BHA.
2663289 Primary Facility 2017-11-21 Drilling/Alteration No Violations Noted Fountain Quail Drilling Rig #8. KOP was at 3508 ft. TD was at 4972 ft. MD on 11/17/17.
2691579 Primary Facility 2018-02-02 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #277 moved onto this well on 1/21/18 and resumed drilling at 4972 ft. KOP was at 2950 ft. (with air rig). EOB was at 9243 ft. MD (7683 ft. TVD). Currently at 22,670 ft. MD and drilling ahead.
2692324 Primary Facility 2018-02-08 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #277. TD was at 24,600 ft. MD (7662.66 ft. TVD) on 2/3/18. The 5 1/2" casing was set at 24,593 ft. on 2/6/18. Started pumping cement and pump on cement truck failed. Backup cement truck began pumping and it failed. There was an attempt to use the rig mud pumps. That attempt failed possibly due to cement setting up. Operator plans to sidetrack this well at a later date.
2705718 Primary Facility 2018-03-13 Routine/Partial Inspection No Violations Noted On 3/14/18 a routine complete inspection of the R. Smith Well Site was conducted in response to a notification of proposed construction activity on the pad. The following was observed:

>>The site was in a high activity phase at the time of inspection drilling appeared to be completed and the site was in apparent transition to post drilling activities. No drilling rig was present on site at the time of inspection.

>>Construction of a proposed AST had not begun at the time of inspection.

>>Many water trucks were present on site in association with the attached impoundment.

2738554 Primary Facility 2018-05-16 Incident- Response to Accident or Event No Violations Noted On 5/16/18 an incident inspection of the R. Smith well site was conducted in response to a spill reported by the operator on 5/14/18. The following was observed:

>>The operator reported that roughly 35 gallons of mixed rain water and produced water was spilled to the pad when a section of containment was moved without being drained first. Fluids were spread over a large area of the pad along the area in front of the production units per discussion with Mike Cromley at the time of inspection. This does constitute a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents prevents pollution to the ground or Waters of the Commonwealth however, the violation is regarded as immediately corrected.

>>Site inspection found no residual evidence of the spill. The operator reported that all visibly impacted gravel from the spill area had been removed (roughly two clamshell dumpsters).

2747589 Primary Facility 2018-06-29 Routine/Partial Inspection No Violations Noted On 6/29/18 a routine partial inspection of the R. Smith Well Site was conducted in response to concerns about runoff raised by a nearby resident. The following was observed:

>>An unstabilized soil pile near the corner of the pad with a newly improved drain. Controls below the pile were absent though the vegetation was sufficient. No violation, remediation recommended.

>>Another only partially stabilized soil pile with no controls below the pile was located along the accessory pad access road. No controls beyond vegetation were present below the soil pile. No violation, monitoring recommended.

>>An AST was present on the accessory pad.

>>The pad was active at the time of inspection with drilling preparations ongoing. A sound wall was present around the pad perimeter.

2762086 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted On 8/2/18 a complaint response inspection was conducted of the R Smith well site in response to reported concerns of inappropriate drainage from the pad. WQSS John Carson accompanied. The following was observed:

>>A top soil pile on the access road to the accessory pad remained lacking vegetation and with spotty temporary stabilization. The vegetative strip below remains in good condition. Remediation recommended, no violation.

>>Emptying partially filled containment is encouraged to aid in identifying any possible leaks. No violation.

>>No evidence of excessive or unpermitted runoff from the pad was found.

>>Box in containment appears to be collecting emergency blowdown. If so it should be properly marked. No violation.

2817218 Primary Facility 2018-12-18 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #277 moved onto this well on 12/15/18 and resumed drilling at 5500 ft. MD KB. KOP was at 2930 ft. (with air rig). Currently at 6325 ft. and still building curve.
