2173558 Primary Facility 2013-06-07 Routine/Complete Inspection No Violations Noted Inspection conducted on 6/7/13 with Bob Wrazen, URS and Dean Jameson, Hess of the Hammond Pad. Inspection based on site visit to discuss pad reclamation requirements. Hess is expecting to pick a contractor to reclaim the pad within the next week. The Department will allow the access road to stay if the land owners so desire. An inspection of the existing silt sock revealed that new silt sock will need to be installed up gradient of the existing silt sock. Plugging is expected to begin of the Hammond 1V within the next couple of weeks. Site is temporarily stabilized at this time. At the owner’s request, stone recovered from the well pad may be placed in a pile along the access road. Silt sock will need to be installed around this stone to capture any stone fines which may mobilize during rain events.

Inspection conducted on 7/2/14 with Robert Wrazen, URS and Dean Jamieson, Hess. Inspection is to confirm Hess’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination. The Department’s determination is that the site is in fact stabilized and meets the criteria necessary for the permit’s termination.
2517880 Primary Facility 2016-09-08 Routine/Complete Inspection No Violations Noted Inspection of the Hess Corp. Hammond 1V plugged gas well in Scott Twp. Wayne County conducted September 8, 2016 at approximately 11:00 am. The lease road for the reclaimed site still exists; it is on Travis Road, .28 of a mile south of the crossroads with Autumn Leaves road. I pulled up the lease road onto the old pad which is now reclaimed. The pad is now a grassy field with no monument for the plugged well. I walked the entire pad, no sign of the plugged well leaking. I did not find anything significant with my metal detector that would signify a buried monument close to ground level. The well was plugged in August of 2013. No Violations
