2253664 Primary Facility 2014-03-14 Incident- Response to Accident or Event No Violations Noted Inspection of a ~2 barrels of "compressor water" spill reported on 3/14/2014 at 13:45. During the inspection the Department observed two plastic tanks one 1000 gallon and the other 500 gallon that the Operator was actively working on upon arrival. The Operator indicated that the tanks appeared to have broken valves and the Operator disconnected the tanks and moved the tanks to an inclined position to stop the leak. The Department observed an area behind the tank with standing water and part of the secondary containment showing. The Department suggests the Operator removed the affected material inside the containments and dispose of the residual waste. The Department also notes that the Operator is compliance with the Departments current spill policy.
2261841 Primary Facility 2014-04-14 Administrative/File Review No Violations Noted Updated inspection # 2257026 from PEND to NOVIO. Operator filed all necessary confirmatory samples and responses. Spill was contained by clay layer and secondary containment inside the earthen dike. Released material did not reach the ground or waters of the commonwealth.
2305809 Primary Facility 2014-03-24 Follow-up Inspection No Violations Noted Follow-up inspection to a spill reported to the Department on 3.14.2014. During the inspection it appears that the Operator has removed the material affected by the spill. In the process the Department observed the plastic containment was only covering the sides of the secondary containment and did not appear to be present on the bottom of the earthen dike. This is contrary to what was observed and reported during the previous inspection. The Department recommends the Operator sample the remaining material to ensure the cleanup has restored the affected are to background samples at the site.
