Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1778626 Primary Facility 2009-03-19 Routine/Complete Inspection Violation(s) Noted Drilling had been completed at the time of the inspection. Well appeared to be equipped for production, brine tank onsite and appears to be in sound condition. The site had been regraded at the time of the inspection, however it had not been seeded or mulched. 1) Noted that the filter fabric fence BMP had not been installed properly. No accelerated erosion & sedimentation was noted onsite or on the access road, however it is possible that these problems may develop due to the exposed nature of the site. 2) Noted that that culverted waterway adjacent to the site along the access road was not properly isolated from disturbed area with BMP's.
1793898 Primary Facility 2009-04-24 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Well appeared to be equipped for production, brine tank onsite and appears to be in sound condition. The site had been regraded at the time of the inspection, however it had not been seeded or mulched. 1) Noted that the filter fabric fence was not installed properly. The bottom of the fence had been folded under toward the site and soil was placed on top of the fence.. No accelerated erosion & sedimentation was noted onsite or on the access road, however it is possible that these problems may develop due to the exposed nature of the site. 2) Noted that the culvert inlet for the waterway adjacent to the site along the access road was not isolated from disturbed area associated with the access road and well pad with erosion control BMP's.
1843706 Primary Facility 2009-10-21 Routine/Complete Inspection No Violations Noted Drilling completed 10/21/2008. There was a violation issued 3/19/09. The site was graded seeded and mulched, however it appears the vegetation only grew in certain areas. Any areas that are lacking vegetation should be limed, fertilized, seeded, and mulched. There is an erosion ditch forming on the left side of the fill slope facing the highway and there is an intermitant stream at the base of the site to be concerned about. Due to the small amount of growing season left the operator should use an alternative method of erosion control until vegetation can take hold. Brine tank is sound and well is in production. The operator should monitor the well site and access road for erosion and sedimentation.
2484739 Primary Facility 2016-05-13 Routine/Complete Inspection No Violations Noted At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Brine tank located on site and appears structurally sound. No E&S issues in relation to site and/or access road.
