Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2001433 Primary Facility 2011-09-06 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Drill pit located onsite and it had more than the required two feet of lined freeboard. E&S controls had been implemented properly and no accelerated erosion or sedimentation was observed in relation to the well-site or access road. Verification that the E&S plan is located onsite could not be made due to the fact the tube could not be opened. The operator should monitor the access road closely to insure no accelerated erosion issues occur with the road due to the current and near future weather.
2010427 Primary Facility 2011-10-12 Drilling/Alteration No Violations Noted Well shut in on 4 1/2" x 2" swage. 7" x 4 1/2" annulus open. Well site restored, seeded and mulched.
2021080 Primary Facility 2011-11-21 Routine/Complete Inspection No Violations Noted Drilling was completed, well was equipped for and in production. Brine tank located onsite and it appeared to be in sound condition. The site had been regraded and vegetation had begun to grow. The vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation observed in relation to the access road or well-site.
2053765 Primary Facility 2012-03-26 Site Restoration No Violations Noted Well appears to drilled and equipped for production. Well apparatus and brine tank located on site appear to be in sound condition. There was no accelerated erosion observed on well pad or access road. The Department suggests the Operator continue to monitor the site until the 70% vegetative coverage criteria met.
2647113 Primary Facility 2017-10-13 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Brine tank within earthen dike and appears to be structurally sound. No E&S issues.
