We used an Oakton MultiParameter PCSTester 35 and a Hanna Soil Test and Electrical Conductivity & Temperature meter to screen the water in the Production Fluid Storage Tanks and Gas Production Unit containment. Conductivity readings were in the range of (700-800) microSiemens/cm in water inside the production fluid tanks containment and less than (500) microSiemens/cm in water inside the GPU containment. We collected a water sample using the PADEP Bureau of Labs Standard Analysis Code 046 in the production fluid tanks containment at N-41.58295; W-076.01905. The parameters to be analyzed are listed in the Department's 11-17-15 Macialek inspection report. The intent of screening the water for conductivity and collecting the sample is to determine if the water meets the requirements of PA Title 25, Section 78.60 and Warren's SOP in the event this containment water will be discharged to stormwater controls on-site. |

At the start of my inspection, I met with Gary Gerard of BKV. I conducted the inspection accompanied by Mr. Gerard. Based on the circumstances described in the findings of this report, I will conduct a Follow-Up Inspection to confirm or deny an incident of a pollution-causing substance (contaminated storm-water) had contacted the well pad surface (constructed of a permeable material) and/or the suspected substance (contaminated storm-water) had been adequately removed and disposed of in a proper manner. The Follow-Up Inspection will be conducted following the conclusion operations and preferably at the time the secondary containment is recovered. The weather conditions are cloudy skies and breezy with temperatures around 70 degrees Fahrenheit. There was no activity at the site. There are (3) wells at the site in the production phase, Ruark East 1H, Ruark East- Ruark South 2H, and Ruark East 11 H. I initially met with Sean Bonnice, and Cory Bradley, well pumpers of J.P. Reilly & Associates. Mr. Bonnice provided me with the following information. "A Service Rig recently moved to the BKV Macialek pad and it is currently on the BKV Mattocks pad. At the end of production tubing, which is at the end of the curve, around (7,000) feet in depth, tubing was pulled to change an x-nipple and to remove a perforated sub. Lubricators have been installed on all (3) wells. The next step is to install plungers on the wells." Matt Mercaldo, contractor manager, and Gavin Chilson, automation specialist both of BKV arrived during my conversation with the Reilly personnel.
