2648818 Primary Facility 2017-10-11 Routine/Complete Inspection Outstanding Violations - Viols Req'd - Arrived on location at 15:00. Weather, raining, 62 degs. The Warren 2H well was the only well present on pad and was in production at time of inspection. Inspection of well showed: Production tree top and production line pressures were being monitored by SCADA press gauges w/ LED readouts. (4) annular vents were present at wellhead. The (5/P) and (9x5) annuli vents were shut in to SCADA press. monitors w/ LED readouts. The (13x9) vent was open and venting to prod. tank w/ a SCADA press. monitor in line. The (20x13) vent was open and venting to atmosphere through a "Vent Check" fitting. The well cellar was full of water, no bubbling was noted at time of inspection. - A gas check performed at the (20x13) vent opening, using an Altair 5X gas meter showed a sustained reading of 5% Gas, 100% LEL. - This sustained presence of gas detected, outside surface casing, in the cemented (20x13) annulus is indicative of a continued violation of PA Code 78a.85.a.5. - Pending the present ongoing gas migration case underway
2657748 Primary Facility 2017-11-07 Routine/Complete Inspection Outstanding Violations - Viols Req'd - Arrived on location at 08:30. Weather, Overcast, cool, 36 degs. The (13x9) and (20x13) vents were shut in to SCADA gauges this am for 72hr PBUT. PBUT to conclude on Friday 11/10/2017. CALLC will record these annuli PBUTs data and share with the Dept. when processed. CALLC will also have Leidos Env. onsite at conclusion of PBUT to collect gas samples off annuli, where present, for isotopic analysis. - The well cellar was full of water, no bubbling was noted at time of inspection. Left site at 09:00.
2658925 Primary Facility 2017-11-09 Routine/Complete Inspection Outstanding Violations - Viols Req'd - Arrived on location at 10:00. Weather, Overcast, cool, 32 degs. The (13x9) and (20x13) vents were shut in to SCADA gauges on 11/7/17 for 72hr PBUT. PBUT to conclude on Friday 11/10/17. The well cellar was full of water, no bubbling was noted at time of inspection.

- Actual annular pressures at time of inspection, ~ 50 hrs. Into PBUT were: (5xT)= 45.7 psi., (9X5)= 2.0 psi., (13x9)= -1.2 psi., (20x13)= 0.26 psi. - CALLC will record these annuli PBUTs data and share with the Dept. when processed.
