2489918 Primary Facility 2016-05-31 Administrative/File Review No Violations Noted This administration inspection is created to document a spill of production fluid that occurred at the site on 5/11/16. The Department received an e-mail in reference to the spill incident from the PADEP Northeast Regional Office Emergency Response Team on 5/17/16 at 10:33 AM. Matt Shope and Frank Hoehle, DEP O&G WQS, and I responded to the spill incident on 5-17-16 at 13:30. There was no activity or personnel at the site during the inspection. It had been apparent the spill had been remediated. Stone was placed over the original pad material on the pad surface impacted by the production fluid spill. Conductivity of water in the production fluid tanks containment structure near the containment wall and one of the tanks was (9) milliSiemens/cm³. I did not record any elevated soil conductivity readings of concern in the general area of the pad surface where the spill occurred. There is staining/discoloration of water and /or fluid on the side walls of the production fluid tanks and the containment lin
