2834549 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted I arrived onsite at 1514 to perform a routine complete inspection of the pad. The temperature was a mild ~38 degrees F with a light wet snow falling from cloudy skies.

The access road and a small circular section of the pad surface had been plowed to facilitate truck access, the rest of the pad surface was covered in an icy slush with excessive ponding in many areas. The area surrounding the wellheads was submerged in over six (6) inches of water rendering the well cellars inaccessible for visual inspection. Repetitive clunking sound coming approximately every 60-90 seconds from area of wellheads/methanol tote. Operator, please inspect the area and perform any required maintenance.
