2693720 Primary Facility 2018-02-01 Routine/Complete Inspection Violation(s) Noted Upon arrival, Weather: Clear, Cold, ~15 degs. (1) well has been drilled on this pad, the Adams Wyo 1H, was producing at time of inspection. Inspection of well showed: Prod. pressures were being monitored by SCADA press. monitor units at prod. tree top and on prod. line. The (5/P) or prod. csg annulus was shut in to SCADA = 1232.5 psi. The (9x5) and (13x9) annuli were plumbed to surface and tied into a common 2" vent line to Prod. Tanks "PT". *The (9x5) vent was shut in to SCADA = 169 psi. The (13x9) vent was venting to atmosphere through a "Vent Check" fitting w/ inline SCADA = -2 psi., gate valve to PT was closed. A gas check performed at the (13x9) "Vent Check" fitting opening showed: 100 % LEL, 100 % CH4. A flow of gas was also detected, indicative of Defective Cement. (see attached pics and violation.)
2694754 Primary Facility 2018-02-12 Follow-up Inspection Outstanding Violations - Viols Req'd Upon arrival, Weather: Overcast, Cold, ~32 degs. (1) well has been drilled on this pad, the Adams Wyo 1H. On location to collect a gas sample off the (13x9) annulus in follow-up to previous insp. of 2/1/18.

A gas check performed at the (13x9) annulus confirmed a sustained presence of gas, 100% LEL, 100% CH4. A gas sample was collected off this annulus at 09:00 for Isotopic analysis. Sample will be handed over to DEP Geologist, Eric Rooney for processing. The 1H well was in production, no production pressures were recorded at time of inspection. Left location at 09:15.

2825440 Primary Facility 2018-12-12 Drilling/Alteration No Violations Noted I arrived at the site 12:43. The weather conditions are cloudy skies, with a slight breeze, and temperatures in the low to mid 30's. Patterson Drilling Rig # 334 is on the location drilling the Adams 101 well. CHK submitted a Drilling resumption After Break of 30 days or more notification to the Department on 11/26/18 at 8:21 AM. The comments in the notification indicate CHK would resume drilling on the Adams 21H well at 2PM. The current drilling depth is (17,321) feet. I met with Will Doby, site company man and consultant of CHK. He provided me with the following specifics related to site activities. "We are on the Adams 21H. We just cemented the production casing last night. We will drill the Adams 23H well next. The projected spud is tomorrow morning at 08:00 hours. The 21H well was drilled with diesel oil based drilling mud from (6,397 to 17,321) feet. The 21H well was spud with municipal water. Some lime and gel was used to get the thick slurry.
