2129717 Primary Facility 2013-01-03 Drilling/Alteration Pending No well name or permit number on site sign.
2151164 Primary Facility 2013-03-21 Routine/Complete Inspection No Violations Noted 20" COND SET. wELL TAG IS WRONG, SAYS NARDINI. IMMEDIATELY CORRECTED.
2160670 Primary Facility 2013-04-23 Drilling/Alteration No Violations Noted
2272024 Primary Facility 2014-05-14 Routine/Complete Inspection No Violations Noted All wells at the P&G location location have been hydraulically fractured. Frac plugs have been drilled out for the Prevost 1H and Ruark 4H wells. Flowback procedures are set to commence shortly for the Prevost 1H and Ruark 4H wells. I recorded (1,300) microSiemens/cm conductivity in water on the frac tank battery containment liner behind the frac tanks in the northwest corner of the Well Pad. A vacuum hose to remove standing water on the surface of the Pad and containment areas of the site, was staged on the frac tank battery containment liner below the site perimeter earthen berm. The liner extends to the top of the berm. There were some tears observed in the liner, but mainly on the liner on the top of the berm, not near or in the standing water.
2513470 Primary Facility 2016-08-24 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Operator reported a release of approximately 5bbls of stormwater to the ground due to a sightglass faliure on a vacuum truck that was offloading the water to a Frac Tank for storage.
2718982 Primary Facility 2018-04-17 Administrative/File Review No Violations Noted Received & reviewed well site restoration report. No violations were noted. If you have any questions, please contact me.
