2208977 Primary Facility 2013-09-12 Routine/Complete Inspection No Violations Noted I surveyed the secondary containment systems, the solids control and handling in the backyard, the site specific PPC plan, Emergency Response Plan and the Nomac Rig 44 Site Specific PPC Plan and Master Containment Plan. I approved of an alternative waste management practice (OG-71) on 8/15/13 for temporary storage and dewatering of drilling mud and water based drilling mud, as well pug mill processing of the cuttings via an on-site pretreatment system. Mr. White informed me that there have been changes in the process of drilling mud dewatering and drill cuttings processing. Chesapeake no longer utilizes Environmental Drilling Solutions for separation of solid and liquid material in the drill cuttings, (dewatering). The OG-71 application lists EDS as the subcontractor provided the dewatering unit. Only (1) centrifuge is used in the process described above instead of (2) as barite recovery is not needed. The centrifuge provided by Peak Energy Services, a Clean Harbors company, extracts the solids from the liquids in the drill cuttings.

2633507 Primary Facility 2017-08-22 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:16. I met with Nick Krut, Environmental Representative of CHK. The intent of the inspection was to respond to the spill incident that occurred at this site on 8/17/17. Mr. Krut stated the following in reference to the spill. "A (130) barrel capacity vacuum truck was in the process of delivering freshwater to this pad to be offloaded into the Modular Aboveground Storage Structure containing water from well development impoundments and water withdrawal sites. The water in this structure is transferred via a pump to a water transfer line on this pad and eventually transferred to the CHK DPH pad to be used during hydraulic fracturing. The truck was parked near the water storage structure when the spill occurred. The hydraulic line failed on the truck near the connection point between the hose and fluid reservoir releasing approximately (20) gallons of hydraulic fluid onto the pad surface. The company responsible for the spill was Fenton Welding Transport of Wyalusing, PA. They mobiliz
