2249798 Primary Facility 2014-02-28 Drilling/Alteration No Violations Noted Orion Drillling Rig #(15) Virgo is the active drilling rig at the location. The current drilling depth is (7,349.63) feet. I observed areas on and off containment at the location of what appears to be small spills and /or leaks. A white powdery substance, most likely polysaccharide, was spilled to the ground surface near the egress ramp and flare stack pipeline in the backyard. The white powdery substance on the ground is also next to a light tower without a method of secondary containment. Outside of the enclosed area where the oil and water based mud tanks are stored on containment in the corner of the Pad, there are small areas of dark staining and residue on the ground surface. It appears as if oil based mud may have leaked from the hoses to the surface of the ground. Mr. Balum informed me of an oil based mud spill or leaks in the hoses from the oil based mud tanks which occurred due to the mud freezing.

The purpose of this inspection was to ensure the two sediment traps onsite had been converted into infiltration berms and an additional infiltration berm installed along the access road.

After performing an administrative review of the plan and a site inspection, I agree that the aforementioned conditions have been fulfilled.

During the inspection, I noticed several of the tops on the annular vent tanks had cracked. Operator, please inspect the area and repair any deficiencies observed.
