2706377 Primary Facility 2018-03-01 Routine/Complete Inspection Pending I arrived at the site at 12:50. The weather conditions are cloudy skies, with temperatures in the mid 40's and a light breeze. In addition to conducting an inspection of the process to clean the Modular Aboveground Storage Structure (MASS) secondary containment liners which were responsible for setting off a radiation alarm at Keystone Landfill, I conducted an inspection of the area on the well pad where the MASS and aboveground pipeline used to transfer water/fluid into the MASS was formerly located. The MASS was located on the cut slope side of the pad. For additional details refer to the Department's 3-1-18 inspection report for Site ID # 763902.

I accepted the Request for Approval of Previously Approved Alternative Waste Management Practices (OG-71) Form B application for the MASS to operate on this pad on 12/29/17. The Previously Approved Alternative Waste Management Practices Information(s) Previous Authorization # in reference to this OG-71 Form B is 1204144, which is associated with the OG-71 Form
