0263673866 | 2015-09-02 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the well stimulation & completion process completed and the well now in production with a gas processing unit on location supporting the well. At the production fluid storage facility a total of 6-400 barrel production tanks are present inside a polymer lined metal dike area. There is currently a total of 5 active wells on this pad. Addressing the restoration- adequate vegetation and erosion control is in place in the affected area by the well site construction. |
0272352337 | 2014-08-07 | Casing String Witnessed | No Violations | The said well was spudded on this day by Rocky Mountain Drillings top hole rig with a 110 x 32 conductor borehole being drilled and 110 of 26 conductor pipe being set and cemented by Stingray cement services using 255 sacks of class A cement with 2% calcium and .25 pound per sack flake with good cement returns to surface. |
0278219852 | 2014-10-16 | Drill / Deepen / Reopen | No Violations | Sidewinder rig 121 is setting a kick plug after not being able to retreive 80 of Bottom Hole Assembely. This consisted of 12.25 hammer bit, drill sub, 8monel drll collar and a 8steel drill collar. The rig ran 5drillpipe at 19.5lb/ft to a depth of 4543-4043 and set a class H +.07%c-sperse1461+.15CR75-LT. Sting- ray set the cement plug. Then displaced down with 85 barrels of water. |
0418240419 | 2015-03-10 | Completion Testing | No Violations | Status check inspection on the well completion process found Stingray Pressure pumping service rigged up on the hole with 22 of 28 stages completed. Nine Energy Perforating service has perforated the 22nd stage with a 15,000# rated capacity completion plug set @ 11,784 and a total of 45 perforations to be shot in the horizontal section between 11,759 and 11,583. The well head rated pressure tested working capacity is 15,000 with Red Dirt rentals master valve and b-section completion isolation tool set on the well head with 2 manual and one hydraulic master valves in place. |
0557643741 | 2014-10-19 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling process found Sidewinder drilling on the hole drilling the 12.250 2nd intermediate well bore at a depth of 6,854, they have successfully deviated around a lost hammer bit at 5,930. |
0574165237 | 2019-03-11 | Production Wells | No Violations | On this day, I found the pad gated with signage at the entrance, and an Emergency Shutdown (ESD) prior to the gate. There are (5) wells setup to be produced by a plunger lift system, (5) vertical separators, and (6) 400-barrel steel production tanks within polymer-lined steel containment. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 500 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 400 psi. I spoke with Gulfport Energy Representative Steve Habig onsite. |
0711911358 | 2014-10-09 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling progress found Sidewinder Drilling rig 121 on the hole preparing to begin drilling the 12.250 2nd intermediate well bore. In review of the recent Blow Out Preventer test showed the blind rams failed the pressure test and no pressure test documentation to where the issue was corrected, the company man Dean Cook did however provide a copy of the company day report showing where the problem was corrected. |
1412262237 | 2014-10-02 | Casing String Witnessed | No Violations | Sidewinder Drilling rig 121 is now on the hole and has drilled the 24 x 327 surface borehole on air with 325 of 18.625 casing ran with a float shoe and collar used along with 4 centralizers. Stingray cement services cemented the casing using 553 sacks of class A 2% calcium .25 pps flake with 50% open hole cement excess ran and the casing pressure tested to 500 psi over lift pressure upon the landing of the plug with good cement returns to surface, the cement mix water was also tested for ph-chlorides and hardness. |
1412423039 | 2014-10-03 | Drill / Deepen / Reopen | Violations Noted | On site inspection and response to a reported fluid spill of 5 barrels of fluid returned to the half round tank out of the well bore from the drilling process. On this day I was contacted at 14:52 pm by Hal Miller with Gulfport indicating he was on the Truax location and they had overflowed the half round tank used in the solidification process. I spoke with District Supervisor Brent Bear and he requested I respond. Upon arrival I observed Sidewinder drilling rig 121 on location drilling the 17.5 1st intermediate well bore at a depth of 1,300 on air and misting with KCL, inspection of the area affected by the spill found the fluid spilled had been recovered and the area pressure washed with a small reported amount leaking off containment but reported as recovered, Chloride tests ran found one off containment 3 x 3 water puddle containing 700 parts per million chlorides, instructions were given as follows- monitor the fluid returns in such a way to avoid future spills- continue to monitor the area and recover any fluid created within the containment- avoid excessive pressure washing unless the fluid created shall be recovered. |
1429626234 | 2015-04-21 | Completion Testing | No Violations | Status check inspection on the well completion process found the well stimulation process now complete and Mercer Well Service work over rig on location drilling out the completion plugs using 10.7# brine to drill and maintained hydrologic pressure control over the well with Great White Companies tubing snubbing unit on the hole along with BTIs work over blow out preventer. The 2.875 production tubing shall be set in the well bore curve section at approximately 60 degrees with an estimated depth to be between 9,500 and 9,850. Buckeye Water Service is on location hauling out the produced water. |

1838838670 | 2014-10-15 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I was accompanied by District Supervisor Brent Bear with the following observed- Sidewinder Rig 121 is on the hole and is trying to free the drill string and 12.250 hammer bit assembly at a depth of 5,930. The rig is working the drill string and is off bottom coming out of the hole and believes to have recovered all of the drill assembly. Sidewinder cement services is standing by to set a 500 class H deviation plug if the bottom assembly is not recovered. |
1986888214 | 2014-10-31 | Drill / Deepen / Reopen | No Violations | Status check inspection on the drilling process found Sidewinder rig 209 on location drilling the 8.5 production borehole at a depth of 13,618 with a current directional azimuth of 328.13 degrees and a true vertical depth of 9,645 using a 12# oil based drilling mud. |
2109080019 | 2014-10-26 | Drill / Deepen / Reopen | No Violations | Status check inspection of the drilling progress found Sidewinder rig 121 on location drilling the 8.5production borehole in the horizontal curve section with a directional azimuth of 231 degrees and a true vertical depth of 9,344 using 11.8# synthetic based drilling mud. |
