0267683163 2015-08-04 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the affected area depicted in the pre-site has been properly restored with proper vegetation and erosion control present, the well site meets ORC 1509.072. Addressing the well head -The well drilling and completion phase is now complete and the well is producing with 2.875 production tubing set in the well bore. There is 3 gas processing units on location one for each existing well along with gas metering equipment. There is a metal type poly lined containment dike supporting the 3-400 barrel and one single 100 barrel production tanks on location. Proper well identification is posted and the location is secured by a 6 woven wire fence with a emergency shut down button located at the entry point to the well pad.
0363798771 2015-06-04 Completion Testing No Violations Status check inspection on the well completion process found Deep Well Services Snubbing Unit on location and pulling the tubing work over string out of the hole after completing the drilling out of the well bore completion plugs. Excell Energy is on location providing Blow Out Preventer and work over pump services along with Tetra Flow Back services. Upon the recovery of the work over string 2.875 production tubing shall be ran into the well bore and set in the horizontal curve at 80 degrees with a total depth of 10,250.
0754095558 2018-03-08 Production Wells No Violations On site inspection to document the well head annular pressures. The 13.375 well head had (0) zero pressure and the 9.625 well head had 1,700 (PSI) pounds per square inch pressure. The production tubing valves were open along with the (ESD) Emergency Shut Down valve. The location has several emergency shut down control points identified at the well pad entrance- at each of the (3) wells total on the well pad - at the production fluid facility and the gas processing units. The well is producing of the tubing head assembly. On this day I spoke with a company HSE representative identified as Bruce Carpenter and we discussed the annular pressure on this well and the rest of the wells on the pad. I have asked the company to monitor and record all annular pressures and address any problematic wells that may have pressures present that could compromise the well head or casing integrity.
1063504741 2016-06-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control and vegetation established . 3-400 barrel capacity storage tanks are on site located in a polymer lined steel containment dike. The well is in production and annular pressure monitoring is in place.

1420039520 2014-12-31 Casing String Witnessed No Violations The said well was spudded on 12/30/2014 by Precision Rat hole Services with a 120 x 30 conductor borehole being drilled and 120 x 24 conductor pipe ran and set. Universal well service is on location to cement the conductor using 305 sacks of type 1 2% calcium .25 pps flake 40% open hole excess cement with good cement returns to surface. The cement mix water was tested and showed 56 parts per million chlorides and a PH level of 7.
1421755085 2015-01-20 Casing String Witnessed No Violations 01/19/15 Notification was received that the company has hit a reported mine void @ 305 to 312, I spoke with my Supervisor Brent Bear and permission was granted to set a 18.625 mine string with notification through E-Mail being sent to the Division permitting section. On 01/20/15 I witnessed the cementing of the mine string with Schlumberger Cement service on location to cement the casing with the bottom balance job being implemented with a 35 barrel 149 sack class A cement mix. The annular side above the mine void was topped off from surface with a initial 37 sack mix spotted and allowed to set before topping the remaining annular volume off. The company men on site were informed that any casing set with a total length of 200 or more must be pressure tested under 1501:9-1-08 (3).
1422057814 2015-01-23 Casing String Witnessed No Violations Administrative report to document the surface casing cement job missed on 01/22/2015 due to being sick. The company man was in contact with me throughout the evening on 01/21/2015 in regards to the upcoming cement job slated for approximately 10:00 pm with the job being delayed due to a mechanical failure in route of a cement truck. The company man notified me @ 12:00 am for a 2:00 am approximate cment job start time. I informed him that I was very ill and would not be out. We then reviewed the job by phone and agreed they would contact me if they needed anything. A 17.5 surface hole was drilled to 1,610 and 1,592 of 61# J-55 casing was ran with a guide shoe and float collar along with proper centralizer placement demonstrated, Schlumberger cement service will cement the casing using 1142 sacks of class A-15.6# 1.21 yield cement with 20 bbls of 6% gel spacer ran ahead of the cement and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug and 30 bbls of reported good cement returns to surface. H&P Rig 531 is on location.
1422894442 2015-02-02 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found H&P drilling rig 531 on location with the vertical stratagaphical test 8.750 well bore being drilled to a TD of 10,350 and Schlumber wire line service is on location to log the well bore. A plug back-kick plug is scheduled to be set on 02/03/15. 3rd party contractors are on location with vacuum trucks recovering water generated on location from recent rainfall.
1422999922 2015-02-03 Production Wells No Violations On site inspection for a 14:00 pm callout to set the bottom hole stratigraphic test plug. Upon arrival I was informed that the Company Engineer had requested additional formation logging and it would be another 9 hours before they were ready to cement. A review of the plugging plan found 2 plugs will be set with a completed estimated top of cement to be 9,035. The plugs will be set through the drill string.
1423142867 2015-02-05 Plug / Plug Back No Violations Bottom hole stratagraphic test hole plug back on active shale well administrative report. The well total drilling depth was reached @ 10,350 on 02/02/2015 with the next 36 hours spent doing geological borehole surveys by Schlumberger well surveys. I was on location @ for a 14:00 pm call out on 02/03/15 to set the bottom hole plug and kick plug but the company reported they had requested additional surveys and would be at least 9 hours with the plugging taking place @ 2:00 am the next morning. A review of all plugs and depth was done and approved prior to my departure on 02/03/15. With all records received.
